清热解毒方对2型糖尿病患者血清炎症因子干预的临床研究

摘　　要：目的探讨清热解毒方对2型糖尿病(T2DM)患者血清肿瘤坏死因子α(TNF-α)、白介素6(IL-6)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、空腹血糖(FBG)及糖化血红蛋白(HbA1C)水平的干预作用。方法将134例T2DM患者随机分为试验组和对照组,试验组给予清热解毒合剂口服治疗,对照组采用盐酸二甲双胍口服治疗,两组疗程均为8周,治疗前后测定TNF-α、IL-6、FINS、HOMA-IR、FBG、HbA1C以及临床症状的变化。结果治疗后两组TNF-α、IL-6、FINS、HOMA-IR、FBG及HbA1C均明显下降,临床症状明显改善;其中试验组的TNF-α、IL-6降低较对照组更为显著(P<0.05),临床主要症状改善更为显著(P<0.05),两组患者FINS、HOMA-IR、FBG及HbA1C治疗前后比较,差异有统计学意义(P<0.05或P<0.01),组间比较差异无统计学意义(P>0.05)。结论清热解毒方可能通过调控炎症因子的水平,改善IR,起到降低血糖的作用。Objective To explore the intervention effect of Qingrejiedu fang for the levels of tumor necrosis factor- α （TNF-α）, interleukin-6（IL-6）, fasting insulin（FINS）,insulin resistance index homeostasis model assessment of insulin resistance（HOMA-IR）,fasting blood-glucose（FBG）and glycosylated hemoglobin（HbA1C） in patients with type 2 diabetes（T2DM）. Methods 134 patients with T2DM were randomly divided into test group and control group.The test group were treated by Qingrejiedu fang,while the control group were treated by metformin hydrochloride.The treatment period of two groups was 8 weeks.The levels of TNF-α ,IL-6,FINS,HOMA-IR,FBG,HbA1C and the changes of clinical symptoms were measured before and after treatment. Results The levels of TNF- α,IL-6,FINS,HOMA-IR,FBG and HbA1C were all markedly reduced and clinical symptoms improved significantly in two groups after treatment. Compared with control group,the levels reduce of TNF- αand IL-6 in test group was more significant（P 〈 0.05）,and the main clinical symptoms improved more significantly（P 〈 0.05）. There were significant differences in the levels of FINS,HOMA-IR,FBG and HbA1C of two groups between before and after treatment（P 〈 0.05 or P 〈0.01）,while the differences were not statistically between two groups（P 〉 0.05）. Conclusion Qingrejiedu fang has the effect of lowering blood sugar by regulating the levels of inflammatory factors and improving insulin resistance（IR） probably.
