基于多层螺旋CT正常内耳前庭空间方位的解剖学研究

摘  要:目的利用多层螺旋CT测量正常内耳前庭空间方位,为进一步研究前庭相关疾病提供基础性数据。方法收集201 1年4月~201 2年7月在北京同仁医院行颞骨64层螺旋CT检查,且符合纳入标准的受检者240例(480耳),按年龄分为4组:儿童组(<18岁),青年组(18岁≤年龄<45岁),中年组(45岁≤年龄<60岁),老年组(≥60岁)。每组60例,男、女各30例。采用后处理工作站(Extended Brilliance^(TM) Workspace,EBW)多平面重建,图像经标准化处理后测量前庭在不同方位的空间角度。结果前庭前后方位外倾角、上下方位外倾角、左右方位后倾角在左右侧别间、男女性别间、不同年龄组间比较P均>0.05,差异无统计学意义。前庭前后方位外倾角、上下方位外倾角、后倾角及95%置信区间分别为:21.21°±5.51°(10.41°~32.01°);45.82°±5.49°(37.76°~56.58°);37.00°±4.98°(27.24°~46.76°)。结论多层螺旋CT结合后处理可定位前庭空间位置,为进一步阐明某些内耳疾病的发生机理提供信息,为前庭相关疾病的影像诊断提供客观依据。OBJECTIVE To measure spatial orientation of vestibule in normal inner ear by mulit-slice spiral CT so as to provide some basic information for further study of the diseases of vestibule. METHODS The patients who underwent temporal bone CT scan were collected from Apr.2011 to Jul.2012. The 240 cases (480 sides)who met the inclusion criteria were divided into 4 groups: the child group (〈 18 years) , the teenager group (18 years ≤ the age〈45 years) , the middle age group (45 years ≤ the age〈 60 years) , the older group (≥60 years) . Through the multiplanar reconstruction of the images in post-processing workstation, the measurements for spatial orientation were performed. RESULTS 1.There was no statistically significant differences in the extraversion angle of the vestibule in anterior-posterior position, superior-inferior position and interior and exterior position in sides, genders and different groups (P〉0.05) . 2. the extraversion angle of the vestibular in anterior-posterior position, superior- inferior position and interior and exterior position and 95% confidence interval were respectively 21.21°± 5.51° (10.41°-32.01°) ;45.82°±5.49° (37.76°-56.58°) ; 37.00°±4.98° (27.24°-46.76°) . CONCLUSION The Volume CT combined with post-processing technology can orient the spatial orientation of vestibular, and is helpful to the objective judgment and diagnosis of vestibular diseases.
