5-氨基酮戊酸光动力疗法治疗肛管内尖锐湿疣的疗效观察

摘　　要：目的探讨5-氨基酮戊酸光动力疗法(ALA-PDT)治疗肛管内尖锐湿疣的临床疗效及护理方法。方法回顾性分析用ALA-PDT治疗的36例肛管内尖锐湿疣患者的临床资料,采用盐酸氨酮戊酸配制温敏凝胶,均匀涂于皮损处,光动力治疗仪照射,能量密度100/cm2。每周一次,并进行治疗前宣教及心理疏导、治疗中有皱折处需涂满药液、治疗后外涂抗生素等护理方法。结果 36例患者,男20例,女16例,经3次治疗治愈22例,6次治疗治愈10例,4例因疣体较大经3次治疗疣体明显缩小,但由于患者经济原因不再坚持治疗。治愈率88.90%,有效率100%;疗程结束后经6个月随访,复发4例,复发率12.5%。不良反应有局部疼痛、糜烂,视情况予加强局部护理、止痛,大多数症状自行消失。结论艾拉-光动力疗法治疗肛管内尖锐湿疣是一种疗效好、复发率低、微创、实用的治疗新方法,治疗过程中需加强护理和临床指导。Objective To evaluate the clinical effect and patient nursing in the treatment of ALA-Photodynamic therapy for intra anal-rectal warts. Methods A retrospective survey and analysis of clinical data,effect and nursing for the treatment of intra anal-rectal warts in 36 patients referred to the STD clinic of the Fourth People＇ s Hospital of Shenzhen. Results Thirty-six patients including 20 males and 16 females. After 3 times of ALA-PDT therapy, warts were cleared in 22 cases and after 6 cycles, 10 cases were cure. The warts of other 4 patients decreased in size and the patients gave up the following treatment. The cure rate was 88.9% and the effective ratio was 100%. After 6 months＇ follow up, 4 cases had recurrence warts and it was 12.5% of the total number of the patients. The side effects of the treatment include pain and erosions after the photody-namic therapy. The proper nursing and using of analgesic can release the symptoms. Conclusion The ALA - PDT is an effective method in reducing the recurrence ratio and that of the time intervals of the whole treatment for intra anal-rectal warts.
