摘 要:目的评估1.5T磁共振时间飞跃法磁共振血管成像(three—dimensional time—of flight magnetic resonance angiography,3D-TOFMRA)和64排螺旋(、TlIIL管成像(computed lomography angiography,CTA)对颅内动脉瘤的诊断价值。方法同顾性分析临床就诊疑似颅内动脉瘤67例患者的MRA、CTA和I)SA图像资料,以I)SA检m动脉瘤数同千¨大小为金标准埘照,比较MRA、CTA对颅内动脉瘤检测的敏感性和特异性,并对MRA和CTA与I)SA测得病灶大小一致性检验。结果43例确诊有颅内动脉瘤,共44个病灶,MRA和CTA对颅内动脉瘤总的诊断准确性分别为88.2%和89.7%,直径〉3F11nl的病灶诊断准确性为96.d%和96.5%;对直径≤3mm的病灶诊断准确性分别为80%和83.9%。MRA和CTA与DSA卡H比对动脉瘤大小测量的桐天系数分别为0.81和0.95。结论MRA和CTA与I)SA测得病灶大小的相关性较好,特别是对3mrn以上的动脉瘤。Abstract:Objective To assess the diagnostic value of three-dimensional time-of-flight magnetic resonance angiography(3D-TOF MRA)at 1.5Tesla scanner and 64-slice spiral computed tomography angiography(CTA)for intracranial aneurysms. Methods MRA, CTA and DSA image data were analyzed retrospectively in sixty-seven patients suspected with intracranial aneurysm clinically. The sensitivity and specificity of MRA and CTA in detecting intracranial aneurysm including the number and size of aneurysm were corn pared with that of DSA, which regarded as the gold standard. The differences in evaluating aneurysms by MRA, CTA and DSA were analysed with consistent test. Results 4;1 aneurysms in 43 patients were detected by DSA and operation. The overall diagnostic accuracies in detecting intracranial aneurysms by MRA and CTA were 88.2% and 89.7% respectively, in which the accuracies were 96.4%and 96.5% for the aneurysms more than 3 mm in size, 80% and 83.9M for aneurysms less than 3 mm in size, respectively. The correlation coefficients were 0.81 and 0.95 in ewduating the size of aneurysms with MRA and CTA, respectively, compared with DSA. Conclusion MRA and CTA both have high consistency to DSA in measuring the size of aneurysms, especially for the an- eurysms with more than 3 mm in size.
