摘 要:目的评价新双源Flash CT诊断肺静脉畸形引流的临床价值。方法对新双源Flash CT诊断为肺静脉畸形引流的21例患者进行回顾性分析。所有病例均行Flash单期心脏增强检查及超声心动图检查,21例患者均有手术结果。记录每位患者的CT及超声结果并与手术结果对照。结果2l例患者,CT诊断完全性肺静脉畸形引流18例,部分性肺静脉畸形引流3例,心上型14例,心内型5例,心下型1例,混合型1例,误诊1例。超声心动图诊断完全性肺静脉畸形引流17例,部分性肺静脉畸形引流0例,心上型10例,心内型6例,心下型0例,混合型l例,误诊1例,漏诊4例,其中部分型肺静脉畸形引流超声(3例)均漏诊。结论新双源FlashCT对肺静脉畸形引流尤其是部分性肺静脉畸形引流的诊断明显优于超声心动图,为患者术前提供充足的信息,值得临床推广应用。Objective To evaluate the clinical value of Somatom Definition Flash CT angiography in diagnosis of anomalous pul- monary venous drainage. Methods 21 children with anomalous pulmonary venous drainage were retrospectively analyzed. All cases underwent Flash CT angiography, echocardiogram and operation. The results of CT and echocardiogram were compared with that of operation in all cases. Results Of all 21 patients,CT showed complete anomalous pulmonary venous connection in 18 cases and par- tial anomalous pulmonary venous drainage in 3 cases, including supracardiac type (n~ 14), cardiac type(n=5), infracardiac type[n =1), and mixed type(n:l). 1 case was misdiagnosed by CT. 17 cases with complete anomalous pulmonary venous drainage were diagnosed by echocardiogram, including supracardiac type ( n= 10 ), cardiac type ( n= 6 ), infracardiac type ( n= 0) , and mixed type (n= 1), 4 cases( included all 3 cases partial anomalous pulmonary venous drainage)were missed and 1 case was misdiagnosed by echo- cardiogram. Conclusion Flash CTA can provide valuable informations for diagnosis of anomalous pulmonary venous drainage, and is superior to echocardiogram especially for partial anomalous pulmonary venous drainage.
