摘  要:目的探讨不同类型及程度肺功能损害的特发性肺纤维化(idiopathicpulmonaryfibrnsis.IPF)溥从MS(、T表现特征及与肺功能检查诸项指标之间的相关性。方法经l临床病理证实的30例IPF患者均行薄层MSCT扫描.观察不同类J魁及程度IPF的薄层MSCT秆象。并对其预定层面各征象累及范嗣进行评分。将薄层MSCT各征象与肺功能渚项指标进行Spearman相关性分析。结果30例IPF均表现为不同程度限制性通气功能障碍及弥散功能障碍。薄层MSCT符征象总’仁定量评分与肺活量(、ritalcapacity,VC;y=0.646,p〈0.001)、肺总量(total lungcapacity,TI。C;r=一0.661.,J〈0.001)、残气量(residual vo1tlne.RV:=一0.415。P=0.023)呈负相火;与用力肺活培(forced inspiratory vitalcapacity,FV{、Ir=-0.624.P〈0.001)、笫1秒川力呼气容积(forced expiratory volume in one second.FEVl;r一一0.483.P一0.007)呈负相关;与一氰化碳弥敞量(diffusion capacity for carbon monoxide of the lung,DI=CO=一0.732,P〈O.O01)、比弥散量(specificdiffusingcapacity.DI。DLVA;r=一0.504.P=0.005)呈负相关;与限制性通气功能障碍分级(r=0.843.P〈O.001)、弥散功能障碍分级(r=0.838,P%0.001)呈正相关。结论不同类型及程度肺功能损害患者薄层MSCT表现不同.薄层MSCT半定量评分能够很好反映啼功能损害的状况。Objective To determine the correlation between thin-section MSCT findings and pulmonary function test results dif- ferent styls and various extemt pulmonary function defect patients with idiopathic pulmonary fibrosis. Methods 30 cases of idio pathie pulmonary fibrosis proved clinically ,rod/or pathologically were presented. All patients underwent thin section MS('T . Thin section MSCT scans were assessed for the presence and the extent of abnormal parenchyma in different styls and various extemt pul monary function defect patients. The correlation between thin section MSCT score and the values of pulmonary function tests was examined by using the Spearman rank correlation coefficient. Results Of 30 cases, the pulmonary function tests of all patients re- vealed various exent restrictive ventilatory and diffuse function defect. The analytic results showed the total scores in d^in-section CT negatively correlated with vital capacity (VC, r-- -- 0. 646, P^0. 001) ,total lung capacity(TLC, r-- --0. 661, P^0. 001 ) ,residual volume(RV, r=--0.415, P 0.023), forced inspiratory vital capacity {FVC, r=--0.624, P^0.001), forced expiratory volume in one second (FEVI,r= --0. 483, P=0. 007), diffusion capacity for carbon monoxide of the lung (DIk'(), r=--0. 739, P^0.001), specific diffusing capacity (DI.C()/VA, r= -- 0. .504 , P=0.005), positively correlated with the staging of ventilatory defect (r=0. 84"a;, P〈'0. 001) and diffuse function defect (r--0. 838, P(0. 001). Conclusion There are different MSCT heteropha nies in different styles and various extent pulmonary function defect of idiopathic pulmonary fibrosis ,and there is good correlation between the scorning system and different styles and various extent pulmonary functiong defect.
