呼出气体冷凝物中肺癌标志物采集装置的改进和优化设计

摘　　要：为提高呼出气体冷凝物(EBC)中肺癌标志物的准确采集,设计一种新型的EBC采集装置,为提高EBC中肺癌标志物的准确分析,我们研究最佳采集温度、最佳采集时间等采集装置的优化工作参数。针对EBC中的肺癌标志物癌胚抗原(CEA),使用设计的装置与国外的EBC采集装置Ecoscreen进行对比分析,实验结果表明,在同样条件下我们的装置采集量多于Ecoscreen、CEA浓度明显高于Ecoscreen,说明该装置更适合采集EBC中的CEA。使用该装置采集了65例肺癌患者与56例健康受试者EBC,发现肺癌患者EBC中CEA浓度明显高于健康受试者,通过受试者工作特征曲线(ROC curve)获得EBC中CEA阳性阈值为2.55 ng/mL,此时敏感性为49.2%,特异性为83.9%。结果表明,本研究设计的新型呼出气体冷凝物采集装置能够很好地采集到肺癌病人EBC中的CEA,具有更高的灵敏度和特异性。In order to precisely collect lung cancer biomarkers in exhaled breath condensate （ EBC ） , a new collection device was designed and the best working parameters were ensured such as the best collection time and the best collection temperature. The device was used to detect CEA （Carcinoma Embryonic Antigen）, a lung cancer biomarker. Compared with commercial EBC collection device Ecoscreen,this device is more suitable to collect CEA in EBC. This device could collect more EBC and the concentration of CEA collected was higher. This device was then used to collect EBC of 65 lung cancer patients and 56 healthy controls. The results showed the concentration of CEA of the lung cancer patients was notably higher than that of the control. The threshold of positivity of CEA in EBC was calculated as 2.55 ng/mL using receiver operating characteristic curve（ ROC curve） , with sensitivity 49.2% and specificity 83.9%. This research showed,the new EBC collection device could better collect CEA in EBC in lung cancer patients, with higher sensitivity and specificity.
