左卡尼汀及其衍生物临床应用的循证证据及评价

摘　　要：通过检索PubMed、Embase、中国生物医学文献数据库、中国期刊全文数据库,搜集左卡尼汀及其衍生物临床应用的相关研究,根据2009年牛津证据分级与推荐意见强度(治疗部分),按证据质量从高到低排序,纳入已有的最高质量的研究,探讨左卡尼汀及其衍生物临床应用的循证证据支持情况。结果显示,左卡尼汀及其衍生物在阿尔兹海默病、透析相关肾性贫血、男性不孕症的治疗方面证据质量较高,但是在许多疾病的治疗方面尚缺乏高质量的证据,仍需进行大规模的临床研究以证实其疗效及安全性。Literatures of clinical application of L-carnitine and its derivatives were systematically searched through PubMed, EMbase, Chinese biological and medical disk （CBMDISC） and China National Knowledge Internet （CNKI）. The quality of the evidence was sorted according to the levels of evidence and strength of recommendations grade system of Oxford Centre for evidence-based medicine, and the highest level of evidence available was enclosed and evaluated to serve as the reference for medication. The quality of evidence of L-carnitine and its derivatives was moderate in Alzheimer＇s disease, dialysis-related renal anemia and male infertility. While, further extensive clinical trials are warranted to prove the efficacy and safety of L-carnitine and its derivatives in the treatment of other diseases.
