摘  要:目的:观察中药复方肾疏宁对腹膜透析相关性腹膜纤维化大鼠腹膜功能、形态、相关蛋白、细胞因子表达的影响,初步探讨其防治作用及可能机制。方法:腹膜透析液+红霉素注射法诱发SD大鼠腹膜透析腹膜纤维化模型,依体质量随机分为空白对照组、低浓度模型组(1.5%腹膜透析液模型组)、高浓度模型组(4.25%腹膜透析液模型组)、低浓度干预组及高浓度干预组,每组24只。低浓度干预组及高浓度组干预组予肾疏宁[按43.93 g.(kg.d)-1]灌胃,对照组和模型组予等量生理盐水灌胃。检测腹透液葡萄糖浓度,计算超滤量(UF)。观察腹膜厚度改变,酶联免疫法(ELISA)检测腹膜组织转分化生长因子-β1(TGF-β1)表达,Western blot法测量α-平滑肌肌动蛋白(α-SMA)的表达。结果:第12周治疗后,低浓度干预组超滤量(-4.21±1.83)mL、腹膜厚度(74.68±15.33)μm、TGF-β1(112.74±22.51)pg.mL-1、α-SMA(0.68±0.39),高浓度组干预组超滤量(-5.83±1.50)mL、腹膜厚度(211.75±58.23)μm、TGF-β1(110.97±16.23)pg.mL-1、α-SMA(0.73±0.24),与同浓度模型组比较,差异有统计学意义(P<0.05,P<0.01)。结论:肾疏宁可改善腹膜超滤量,抑制腹膜纤维化的进展,其作用机制可能通过延缓腹膜相关细胞间充质细胞转分化(EMT)进程来实现。Objective : To observe the effect of compound Shenshuning on the expression of rats with peritoneal fimction, morpholo- gy, protein, cytokine peritoneal dialysis-related peritoneal fibrosis, to investigate its preventive effect and possible mechanism. Methods: SD model rats with peritoneal fibrosis induced by peritoneal dialysate and ery thromycin injection according to their weight were randomly divided into blank control group, model group, low concentration model group (1, 5% peritoneal dialy- sate) , high concentration model group (4.25% of peritoneal dialysate) , low concentration intervention group and high concen- tration group, 24 rats in each group. Low concentration intervention group and high concentration intervention group intervention group received Shenshuning [ 43.93 g · (kg · d) ^-1] by garage, control group and model group were given normal saline. Tbe glucose cnneentration of peritoneal dialysate was detected and uhrafihration (UF) calculated. The change of peritoneal thickness was observed, transdifferentiation growth factor β1 (TGF- β1) expression of peritoneal tissue was detected with enzyme linked immunosorbent assay ( ELISA ) , the expression of alpha smooth muscle actin ( α - SMA ) was measured with Western blot meth- od. Results: The twelfth weeks after the treatment, the ultrafiltration volume ( -4.21 ± 1.83) mL, peftoneal thickness (74.68 ± 15.33) μm, TGF - β1 ( 112.74 ±22.51 ) PG · mL^-1, α - SMA (0.68 ± 0.39 ) of low concentration intervention group and the ultrafiltration ( - 5.83±1.50) mL, peritoneal thickness (211.75 ± 58.23) μm, TGF - β1 ( 110. 97 ± 16.23) PG·mL-1, α -SMA (0.73±0.24) of high concentration intervention group statistically had significant difference ( P 〈 0.05, P 〈 0.01 ) compared with the same concentration of model group.
