摘  要:目的探讨膜突蛋白(moesin)及moesin抗体与血管内皮损伤相关疾病的临床相关性。方法利用纯化的重组moesin作为抗原建立间接ELISA,在实验中测定抗原最佳包被浓度、抗体最佳稀释浓度及酶标二抗最佳稀释倍数等。采用酶联免疫吸附法检测120例血管内皮损伤相关疾病[急性冠脉综合征(ACS)20例、原发性高血压30例、颈动脉硬化20例、糖尿病20例及高脂血症30例]患者及健康对照组120例健康体检者血清中抗moesin抗体。结果抗moesin抗体在120例血管内皮损伤相关疾病的阳性检出率较高,达25%~70%,显著高于正常对照组的5%(χ2=55.679,P<0.01);其中ACS组阳性检出率最高达70%,明显高于其他疾病组(χ2=11.774,P<0.05),其他各亚型疾病组之间差异无统计学意义(χ2=1.731,P>0.05)。结论抗moesin抗体在血管内皮损伤相关疾病的患者中有较高的检出率,虽然在伴有单一的导致血管内皮损伤危险因素的高血压、高血脂、糖尿病的患者中抗moesin抗体阳性检出率稍低,但在这些危险因素的协同作用下引起的ACS患者中抗moesin抗体阳性检出率较高。提示抗moesin抗体对于诊断血管内皮损伤,作为一个全新的诊断线索有一定的临床价值。Objective To investigate the moesin protein and anti-moesin antibody as well as the clinical correlation to vascular endothelial injury related diseases. Methods In the experiement, the purified reconstruction of the moesin-protein as antigens was used to establish an indirect ELISA to determine the optimal coating concentration of antigen, best diluted concentration of antibody, and the two of the best diluted times. Elisa-linked immunosorbent assay was used to detect vascular endothelial injury related diseases in 120 cases (20 cases of ACS 30 cases of primary hypertension, 20 cases of carotid atherosclerosis, 20 cases of diabetes and 30 cases of hyperlipidemia) and in the control group of 120 healthy people with serum of anti-moesin antibodies. Results The anti-moesin antibodies in 120 cases of vascular endothelial injury related diseases had a positive rate of 25% -70% , which was significantly higher than the 5% in the control group (X^2 =55. 679,P 〈0. 01). The positive rate of the ACS group reached 70%, the highest of all disease groups (X^2 = 11. 774, P 〈 0. 05 ) while the other subtypes diseases showed no significant difference (X^2 = 1. 731, P 〉 0. 05 ). Conclusions The anti-moesin antibody in patients with vascular endothelial injury related diseases showed high positive rate. With a single cause of endothelial injury risk factor in patients with hypertension, hyperlipidemia and diabetes, the anti-moesin antibody positive rate was slightly lower. However, the synergistic effect of these risk factors induced higher anti-moesin antibody positive rate in ACS patients. The new indication of the anti-moesin antibodies in the diagnosis of vascular endothelial injury has provided definite clinic value for further examination and discussion.
