影响儿童过敏性紫癜复发临床相关危险因素的回归分析

摘　　要：目的调查分析影响儿童过敏性紫癜(HSP)复发的临床相关危险因素。方法收集2010年1月至2011年12月于安徽省立儿童医院住院确诊为HSP患儿256例的临床资料,并根据患儿病情分为复发组和未复发组,建立数据库,应用回归分析方法分析临床相关危险因素。结果单因素分析显示,复发组与未复发组的年龄、白细胞数、中性粒细胞比例、淋巴细胞比例、单核细胞数、血小板(PLT)、血小板平均体积(MPV)、白蛋白、乳酸脱氢酶(LDH)、尿素氮、肌酐、三酰甘油、总胆固醇、活化部分凝血酶时间、免疫球蛋白G(IgG)差异有统计学意义(P<0.05);Logistic回归分析显示,年龄、单核细胞数、LDH、IgG、PLT、MPV因素中有统计学意义(P<0.05)。结论年龄、单核细胞、LDH、IgG、PLT、MPV因素是HSP复发的危险因素,对于有上述因素的HSP患儿应加强观察。Objective To investigate and analyze the clinical risk factors which affect the recurrence of the children＇s Henoch-Schonlein Purpura（HSP）. Methods The clinical data of 256 cases diagnosed as HSP hospitalized in Anhui Provincial Children＇s Hospital from Jan. 2010 to Dec. 2011 were collected. Based on the disease condition, they were divided into relapse group and no relapse group. Database was established and regression analysis method was applied to analyze the clinical risk factors. Results The univariate anal- ysis showed that there was a statistical difference（ P 〈 O. 05 ）between the two groups in age, leucocyte num- ber, neutrophils ratio, lymphocyte ratio, monocyte number, platelet （ PLT ） , mean platelet volume （ MPV ） , albumin,lactate dehydrogenase （ LDH）, urea nitrogen, creatinine, triglyceride, total cholesterol, activated par- tial thromboplastin time （APTY） , and immunoglobulin G （IgG）. Logistic regression analysis showed that age, mononuelear cell, LDH, IgG, PLT, MPV had statistically significant influence on the recurrence （ P 〈 0.05 ） and were identified as risk factors. Conclusion Age, mononuclear cells, LDH, IgG, PLT and MPV are the risk factors of HSP recurrence,observation on HSP children with the above factors should be strengthened.
