阵发性室上性心动过速射频消融治疗后的左室功能及结构研究

摘　　要：目的:探讨射频消融治疗阵发性室上性心动过速的临床疗效,及对左室功能及其结构的影响。方法:2009年7月至2012年7月,收集采用射频消融治疗阵发性室上性心动过速的患者50例和采用药物治疗的患者50例,比较二者治疗前后的左室功能及其结构变化。结果:两组患者心功能及24h室性期前收缩频数均好转,但是观察组患者改善更明显,差异有统计学意义(P<0.05)。观察组患者临床总有效率90.00%,显著高于对照组的58.00%,差异有统计学意义(P<0.01)。治疗后6个月,两组患者与治疗前比较LVEF水平升高,LVESD、LVEDD、IVSd及LVPWd水平降低,差异有统计学意义(P<0.05);但是观察组患者较对照组改善幅度更显著(P<0.05)。结论:射频消融治疗阵发性室上性心动过速,可以显著提高临床有效率,改善患者左室功能和心肌结构重构,有效缓解患者临床症状。Objective： To explore the clinical therapeutic effect of radio frequency current ablation on paroxysmal superventricular tachycardia and the effects on left ventricular function and the structure. Method： Between July of 2009 and July of 2012, 50 patients with paroxysmal superventricular tachycardia treated with radio frequency current ablation and 50 patients with paroxysmal superventricular tachycardia treated with drugs were collected, and the changes in left ventricular function and structure before and after the treatments were compared. Result： In both groups, the heart function and 24 hours premature ventricular beats frequency were both improved, but those in the observation group were improved more significantly, and the difference was statistical significance （ P〈0.05 ）. The clinical total effective rate in the patients of the obser- vation group was 90.00% , which was significantly higher than that of the control group （ 58.00% ） , and the difference was of statistical significance （ P〈0. 01）. 6 months after treatment, compared with those before treatment, in both groups, LVEF levels were increased, LVESD, LVEDD, IVSd and LVPWd levels were decreased, and the differences were statistical significance （P〈0.05） ; but those indexes in the observationgroup were improved much more significantly than those in the control group （ P〈0. 05）. Conclusion： Using radio frequency current ablation to treat paroxysmal superventricular tachycardia can significantly elevate clinical effective rate, improve the left ventricular function and. myocardial structure reconstruction of the patients and effectively release the clinical symptoms of the patients.
