不同内伤基础甲型H1N1流感的中医病因病机初探

摘　　要：目的:探讨不同内伤基础甲型H1N1流感的中医证候学及病因病机特点。方法:采用回顾性病例研究的方法,比较分析770例甲流确诊病例中无内伤基础病组和5类不同内伤组的中医相关临床资料,归纳各自的证候特点及病因病机。结果:①肺系内伤组咳痰急性加重,肺部啰音、咽部充血突出,危重症发热不显著,多见咳痰、脉数、胸闷憋气、气短;②心系内伤组咽部充血、咽痛及胸闷憋气、倦怠乏力等感觉显著;③肾与膀胱内伤组突出表现为胸闷憋气、湿啰音、痰中带血;④消渴内伤组喘促、憋闷、气短较显著;⑤肝胆内伤组轻者表现为咳痰、肌肉酸痛、腹泻,重者主要见尿黄、倦怠乏力。结论:不同内伤基础对于甲型H1N1流感临床特征及预后具有影响。Objective： To explore syndrome characteristics and etiology and pathogenesis in TCM of H1N1 influenza on different internal injury basis. Methods： We concluded retrospective syndrome characteristics and etiology and pathogenesis on basis of comparing and analyzing 770 definite HIN1 cases which were divided into no internal injury group and five internal injury groups on retrospective study. Results： ①Lung system internal injury group mainly manifested as acute exacerbation, rule and pharyngeal hyperemia, while the main symptoms in critical illness were expectoration, rapid pulse, labored short breathing and less fever; ②Cardiac system internal injury group manifested as pharyngeal hyperemia, pharyngodynia, labored breathing, fatigue; ③Kidney and bladder internal injury group manifested as labored breathing, moist tale and bloody sputum;④Xiaoke internal injury group manifested as wheezing and labored short breathing; ⑤Hepatobiliary system internal injury group manifested as exacerbation, muscle soreness and diarrhea in the mild cases, and yellow urine, fatigue in the severe cases. Conclusion： Clinical features and prognosis of HIN1 were affected by different internal injuries.
