小剂量红霉素治疗婴幼儿支原体肺炎的探索

摘　　要：目的:比较小剂量红霉素与阿奇霉素序贯治疗婴幼儿支原体肺炎的疗效。方法:按区组随机抽样方法将186例婴幼儿支原体肺炎患儿分为治疗组和对照组各93例。两组在综合治疗的基础上,治疗组给予红霉素10 mg/(kg.d)静脉滴注,每天1次,连用10 d,10 d后改为依托红霉素10 mg/(kg.d)口服,每天2～3次,疗程3周;对照组给予阿奇霉素10 mg/(kg.d)静脉滴注,每天1次,7 d后停4 d,接着口服阿奇霉素干混悬剂10 mg/(kg.d),每天1次,口服3 d停4 d,疗程3周。结果:治疗组总有效率92.47%,对照组总有效率89.25%,两组比较差异无统计学意义(χ2=0.583,P>0.05)。治疗组不良反应发生率17.20%,对照组不良反应发生率18.28%,两组比较差异无统计学意义(P>0.05)。结论:小剂量红霉素治疗婴幼儿支原体肺炎的疗效与阿奇霉素相当,不良反应轻微,依从性较好,值得临床借鉴及进一步观察研究。Objective： To observe the clinical efficacy of sequential therapy of low-dose erythromycin and azithromycin for infants with mycoplasma pneumonia. Methods： One hundred and eighty-six children with mycoplasma pneumonia were randomly divided into a treatment group and a control group equally. On the basis of the comprehensive treatment, the treatment group received erythromycin intravenous infusion 10 mg/（ kg d）, once a day. Ten days later they were switched to erythromycin estolate orally 10 mg/（ kg d）, 2 -3 times per day for three weeks. The control group was given azithromycin intravenous infusion 10 mg/（ kg d）, once a day, for seven days. When azithromycin intravenous infusion was stopped for four days, oral azithromycin stem suspension agent was followed. The dose was 10 mg/（ kg d）, once a day for three days with a discontinuation for four days; the treatment course was three weeks. Results： The total effective rate in the treatment group was 92.47% and in the control group was 89.25%, the difference was not statistically significant （x2 =0.583, P〉0.05 ）. The adverse reaction rate of the treatment group was 17.20%, and the control group was 18.28%, the difference was not statistically significant. Conclusions： The effect of Low-dose erythromycin therapy is similar to that of the azithromycin therapy for mycoplasma pneumonia in infants. This therapy with mild side effects and good patient compliance is worthy of clinical reference and further observation.
