Sysmex XE-2100全自动血液分析仪复检规则运用的探讨

摘　　要：目的采用Sysmex XE-2100全自动血液分析仪对专家推荐的23条复检规则进行评估验证,并制定出适合本院患者人群使用的复检规则。方法对仪器检测的1 229份血液标本,同时推血片、染色、镜检,使用专家推荐的23条复检规则,并与之比对,根据假阳性和假阴性情况对23条复检规则进行适当的修改。结果按专家推荐的23条复检规则检测了700份标本,结果为真阳性率23.00%,假阳性13.28%,真阴性率61.00%,假阴性率2.71%,推片率36.28%;用修改后的20条复检规则检测529份标本,结果为真阳性率24.76%,假阳性率3.59%,真阴性率69.37%,假阴性率2.27%,推片率28.35%。结论根据所用仪器性能特点、结合本地区的实际情况,制定合理适用的血细胞计数和涂片的20条复检规则,既能保证检验质量,又可提高工作效率。Objective To assess the validation of 23 review criteria recommend by experts using Sysmex XE-2100 auto- matic hematology analyzer so as to develop a set of suitable re-examination rules for hospital patients. Methods 1229 blood samples having being examined by instrument were selected. Their pushing slides, blood smears and staining were prepared for microscopic examination. The outcomes were compared with the 23 review criteria recommended by experts. Appropriate modifi- cations of the criteria were then made based on false positive and negative results. Results 700 samples were tested using the 23 review criteria recommended by the experts, of which 23. % were the true positive, 13.28% the false positive rate,61. % the true negative,and 2.71% the false negative. The rate of pushing slice was 36.28%. In contrast,529 specimens were tested using the modified 20 rules. The result showed a true positive rate of 24.76% , a false positive rate of 3.59% ,69.37% for the true negative and 2.27% for the false negative. The rate of pushing slice was 28.35%. Conclusion The suitable modified 20 re-examination rules for blood counting and smear based on the characteristics of the instrument and the regional practical situ- ation can ensure the quality of the test and improve work efficiency.
