益髓生血颗粒对α-血红蛋白H病患者造血细胞因子活性与表达的影响

摘　　要：目的观察益髓生血颗粒治疗α-血红蛋白H(α-HbH)病的疗效并探讨其分子机制。方法采用益髓生血颗粒治疗α-HbH病患者25例,治疗3个月后观察临床疗效,并检测治疗前后患者中医证候评分、外周血血红蛋白(Hb)、血小板(PLT)、网织红细胞(Ret)、血红蛋白H(HbH)水平,及外周血中造血细胞因子干细胞因子(SCF)、白细胞介素3(IL-3)、集落刺激因子(GM-CSF)活性与表达的变化。结果患者潮热盗汗、心悸气短、咽干治疗后1、2、3个月与治疗前比较均有明显改善(P<0.05或P<0.01)。咽干治疗后3个月与治疗后1、2个月比较也有明显改善(P<0.01)。面色萎黄、腰膝酸软、眩晕、耳鸣与治疗前比较无明显改善(P>0.05)。患者Hb、HbH水平在治疗后2、3个月与治疗前比较均有提高(P<0.05或P<0.01),Hb治疗后2、3个月与治疗后1个月比较也有明显提高(P<0.01)。RBC、PLT水平治疗后无明显上升(P>0.05)。治疗后3个月,SCF和GM-CSF活性与表达较治疗后无明显增强(P>0.05);IL-3活性与表达治疗后比治疗前则明显减弱(P<0.05)。结论益髓生血颗粒治疗α-HbH病疗效肯定,其疗效的分子机制可能与调节患者外周血造血细胞因子IL-3的活性与表达有关。Objective To observe the therapeutic effect and molecular mechanism of Yisui Shengxue Granules （granules to boost marrow and engender blood） on a-hemoglobin H （α-HbH） disease. Methods Totally 25 patients with α-HbH disease in Guangxi Zhuang Autonomous Region were treated with Yisui Shengxue Granules. The therapeutic effect was observed after 3 months of treatment. The levels of hemoglobin （Hb）, platelet （PLT）, reticuloeyte （Ret） and HbH were detected before and after treatment. The activity and expression of stem cell factor （SCF）, interleukin 3 （IL-3） and granulocyte-macrophage colony-stimulating factor （GM-CSF） was observed before and after treatment. Results There were 24 effective patients and the symptoms such as hot flushes, night sweats, palpitations, shortness of breath and dry throat were significantly relieved after 1, 2 and 3 months of treatment （P〈0.05）. Comparing with after 1-month or 2-month treatment, dry throat was significantly relieved after 3-month treatment （P〈0.01）. There was no significant change after treatment in pale complexion, limp aching lumbus and knees, dizziness and tinnitus （P〉0.05）. The revels of Hb and HhH were significantly increased after 2 or 3 months of treatment （P〈0.05 or P〈0.01）. Comparing with 1 month after treatment, the levels of Hb and HbH were significantly increased after 2 or 3 months of treatment （P〈0. 05 or P〈0.01）. There was no significant change after treatment in the levels of red blood cell （RBC） and PLT （P〉0.05）. The activity and expression of SCF and GM-CSF was not increased after treatment in t （P〉0.05）. The activity and expression of IL-3 was significantly decreased after treatment （P〈0.05）. Conclusion Yisui Shengxue Granules is effective for α-HbH disease and its molecular mechanism maybe relevant to regulating the activity and expression of SCF, GM-CSF and IL-3.
