摘 要:目的研究扶正剔邪搜络方对肺纤维化大鼠不同时相肺功能的影响。方法将128只SD大鼠随机分为假手术组、模型组、扶正剔邪搜络组、泼尼松组,每组32只。气管内注射博莱霉素(0.25ml/100g)建立肺纤维化模型,假手术组注射等体积生理盐水。造模24h后,扶正剔邪搜络组给予扶正剔邪搜络方0.74g/(kg·d),泼尼松组给予5mg/(kg·d),假手术组和模型组给予等体积生理盐水。给药后第7、14、28、45天,观察各组大鼠肺组织病理结果及吸气阻力(Ri)、肺顺应性(Cl)、用力肺活量(FVC)、第0.2秒末呼出容积(FEV0.2)、呼出气量为25%~75%肺活量时平均流量(FEF25-75%)变化。结果模型组第7天肺间质有不同程度炎症,第14天肺间质胶原纤维明显增多,第28天肺纤维区较14天增大、增多,第45天现"蜂窝肺"改变。扶正剔邪搜络组和泼尼松组肺泡炎与纤维化程度均轻于模型组。与模型组同时间比较,扶正剔邪搜络组第14天时Cl、FVC均明显升高(P<0.01),第28天时Ri、Cl、FVC、FEV0.2、FEF25-75%差异有统计学意义(P<0.01),第45天时Ri、Cl、FEV0.2、FEF25-75%差异有统计学意义(P<0.05或P<0.01)。与泼尼松组同时间比较,扶正剔邪搜络组45天时Ri明显降低(P<0.01),CI明显升高(P<0.01)。结论扶正剔邪搜络方可改善肺纤维化大鼠肺功能,早期介入并维持一定疗程作用最明显。Objective To study the effect of Fuzheng Tixie Souluo Formula (Right-Supporting, Evil-Dispelling and Network Vessels-Tracking Formula, FTSF) on the lung function in rats with pulmonary fibrosis. Methods Totally 128 SD rats were randomized into sham operation group, model group, FTSF group and Prednisone group, with 32 in each. The pulmonary fibrosis models were established with intratracheal injection of bleomycin 0.25 ml/100 g. The sham operation group was injected with equal volume of normal saline. The FTSF group and Prednisone group was given 0.74g/(kg · d) FTSF and 5mg/(kg · d) Prednisone respectively, and the sham operation group and model group were given equal volume of normal saline 24 hours after modeling. The lung pathology results, inspiratory resistance (Ri), lung compliance (CI), forced vital capacity (FVC), forced expiratory volume in 0. 2 second (FEV0.2) and forced expiratory flow between 25% and 75% of FVC (FEF25-75%) were observed on the 7th, 14th, 28th and 45th day after administration. Results There were varying degrees of inflammation in pulmonary interstitial on the 7th day, significantly increased collagen fibers on the 14th day, increased pulmonary fibrosis area on the 28th day and honeycomb lung on the 45th day in the model group. Comparing with the model group, the degrees of alveolitis and fibrosis in the FTSF group and Prednisone group were slighter. Comparing with the model group, the C1 and FVC was significantly increased on the 14th day in FTSF group (P〈0. 01). There were significant differences between the model group and FTSF group in Ri, C1, FVC, FEV0.2 and FEF25-75M on the 28th day (P〈0.01) and Ri, C1, FEV0.2 and FEF25-75% on the 45th day (P〈0.05 or P〈0. 01). Comparing with Prednisone group, Ri was significantly decrease (P〈0.01) and CI was significantly increased in FTSF group (P〈0.01).
