18F-FDG PET/CT显像在淋巴瘤诊断、分期和疗效判断中的价值

摘　　要：目的探讨正电子发射型计算机断层扫描技术(PET/CT)显像在淋巴瘤诊断、分期和疗效评价中的价值。方法回顾性分析30例经病理学确诊的淋巴瘤患者,将治疗前后的PET/CT影像结果进行对比分析。结果治疗前27例患者的PET/CT显像与单纯CT发现的病灶位置大部分一致,未影响临床分期;3例患者PET/CT显像发现单纯CT显像未检出的病灶,临床分别由原来的Ⅱ、Ⅱ、I期提高到Ⅳ、Ⅳ、Ⅱ期,改变了进一步治疗方案。霍奇金淋巴瘤患者治疗后改善组与进展复发组比较无明显差异;非霍奇金淋巴瘤患者治疗后改善组与进展复发组比较,SUV max值有明显差异(5.81±3.54 vs 10.34±5.81,P<0.05);30例患者治疗前后相同部位ROI区SUV max值的改变差别有统计学意义。结论 8F-FDG PET/CT显像对于淋巴瘤诊断分期和疗效评价具有重要的临床指导意义。Objective To investigate the clinical value of PET/CT imaging in the diagnosis, staging and treatment of lymphoma. Methods 30 patients with lymphoma pathological proved were studied retrospectively,the PET/CT imaging results before and after the therapy were compared and analysized. Results Before the therapy ,27 patientsPET/CT imaging was nearly the same with CT imaging, the clinical stages were unchanged;3 patientsPET/CT imaging was different with CT imaging,the stages were ascended from Ⅱ, Ⅱ ,Ⅰ to Ⅳ, Ⅳ, Ⅱ. There wasnt obvious statistical difference between the improvement group and the progress and recurrence group of HL pa- tients, but there was obvious statistical difference between the improvement group and the progress and recurrence group of NHL pa- tients, and there was obvious statistical difference between the SUV max of the same region before and after the therapy. Conclusion 18F-FDG PET/CT imaging has significant clinical value in the diagnosis, staging and treatment evaluation of lvmDhoma.
