摘　　要：目的通过对2008～2012年军事医学科学院附属医院抗菌药物使用情况的调查分析,评价该院近5年抗菌药物的应用状况和用药合理性。方法利用计算机管理软件检索2008～2012年军事医学科学院附属医院抗菌药物的品种、剂型、使用量和用药金额,并进行分类、排序和汇总;应用限定日剂量(DDD)法统计并计算抗菌药物的用药频度(DDDs)、日均费用(DDDc)和用药金额与DDDs的排序比,分析临床用药情况。结果军事医学科学院附属医院抗菌药物使用以注射剂型为主,年销售额占总用药金额比例最高达18.57%,抗真菌药使用金额最高,头孢菌素类药使用率最高;2011年该院抗菌药物使用呈明显调整,次年用药金额显著降低,其占总金额比例降至13.70%,用药频度显著下降,药品用量和结构均有较大变化。结论军事医学科学院附属医院抗菌药物使用基本合理,干预效果显著,但仍存在问题,须持续规范其使用,以确保临床用药更加安全、有效。Objective To evaluate the statusquo and rationality of the use of antibiotics in the Afflicated Hospital of Academy of Military Medical Sciences between 2008 and 2012.Methods The categories,specifications,consumption quantity and consumption sum of antibiotics in the Afflicated Hospital of Academy of Military Medical Sciences were retrieved by means of computer management software and the clinical use of antibiotics was analyzed using the method of defined daily dose（DDD）.Results The injection of antibacterials was the leading formulation in the Afflicated Hospital of Academy of Military Medical Sciences.The percentage of consumption sum of antibacterials increased to 18.57% in 2010 but decreased to 13.70% in 2012.The consumption sum of antifungal agents and the DDDs of cephalosporins ranked the first respectively in all antibacterials.There were significant changes in drug use structures,the DDDs and consumption quantity after 2011.Conclusion Antibiotics in the Afflicated Hospital of Academy of Military Medical Sciences are rationally used and the regulatory measures are generally effective,but they are far from perfect.The use of antibiotics should be standardized further to ensure safe and effective drug use.
