摘 要:目的调查泛耐药鲍氏不动杆菌获得性耐药相关基因和可移动遗传元件的关系以及菌株间的亲缘关系,为实时监测和有效控制医院感染的发生提供重要依据。方法收集南通大学附属医院2011年1-4月住院患者标本中分离的泛耐药鲍氏不动杆菌共20株,采用PCR法和测序法检测59种耐药相关基因及13种可移动遗传元件的遗传标记,并对检测结果进行指标聚类分析和样本聚类分析。结果 20株泛耐药鲍氏不动杆菌共检出9种获得性耐药基因、7种遗传标记以及3种管家基因carO、gyrA、parC均存在突变;指标聚类分析提示9种获得性耐药基因与可移动遗传元件关系密切;样本聚类分析提示2、6、9、12、16、17、18、20号菌、1、3、4、5、7、8、10、11、13、14、15、19号菌均为克隆株。结论泛耐药鲍氏不动杆菌携带的相关耐药基因可能由可移动遗传元件介导,同一克隆菌株可引起病区内或病区间的医院感染。OBJECTIVE To investigate the correlation between acquired resistance-related genes and mobile genetic elements in pandrug-resistant Acinetobacter baumannii,and to explore phylogeny among the strains,so as to provide important basis for real-time monitoring and effective control of nosocomial infections.METHODS Totally 20 strains of pandrug-resistant A.baumannii were collected from Affiliated Hospital of Nantong University during Jan 2011 to Apr 2011.59 kinds of resistance genes and 13 kinds of genetic markers of mobile genetic elements were analyzed by PCR and sequencing.In addition,the index and sample cluster analysis was performed.RESULTS In 20 strains of pandrug-resistant A.baumannii,9 kinds of acquired resistance genes,7 kinds of genetic markers of mobile genetic elements were detected and 3 kinds of housekeeping genes carO,gyrA and parC had mutations.In this group of pandrug-resistant A.baumannii,index cluster analysis showed that correlation might exist between 9 kinds of acquired resistance-related genes and genetic markers of mobile genetic elements.Moreover,sample cluster analysis showed that there were clonal transmission of strains 2-6-9-12-16-17-18-20 and strains 1-3-4-5-7-8-10-11-13-14-15-19.CONCLUSION Acquired resistance-related genes in this group of pandrug-resistant A.baumannii may be mediated by mobile genetic elements,the same clone strain can cause nosocomial infections in the same ward or among different wards.
