老年患者医院获得性肺炎易患因素分析

摘　　要：目的探讨老年患者医院获得性肺炎(HAP)的易患危险因素,为减低老年患者HAP的发生率、死亡率提供参考依据。方法 1246例老年患者住院期间发生HAP的100例为观察组,随机抽取同期住院的老年患者100例为对照组,回顾性分析老年患者HAP的易患相关因素。结果观察组的卧床率、留置胃管、留置深静脉导管、留置导尿管的比例,均高于对照组(P<0.05),观察组与对照组的血浆白蛋白水平分别为(33.81±5.15)、(37.32±4.53)g/L(P<0.01);血红蛋白水平分别为(107.47±19.01)、(123.02±15.98)g/L,观察组均低于对照组(P<0.05)。结论老年患者HAP的发生率高,长期卧床、留置胃管、留置导尿管、留置深静脉导管、低白蛋白血症、贫血为HAP的易患因素;控制易感因素,有利于降低老年患者HAP的发生率。OBJECTIVE To observe the risk factors for hospital-acquired pneumonia（HAP） in elderly patients so as to reduce the incidence of HAP and the mortality of the elderly patients.METHODS A total of 100 elderly patients with HAP among 1246 hospitalized elderly patients were chosen as the observation group,and in the same period 100 hospitalized elderly patients were randomly selected as the control group,then the predisposing factors for HAP in the elderly patients were retrospectively analyzed.RESULTS There was a higher proportion of indwelling gastric tube,indwelling deep venous catheter,indwelling urethral catheter,and higher bedridden rate in the observation group than in the control group（P0.05）.The plasma albumin level was（33.81±5.15）g/L in the observation group,（37.32±4.53） g/L in the control group（P0.01）.The hemoglobin level was（107.47±19.01）g/L in the observation group,（123.02±15.98）g/L in the control group（P0.05）.CONCLUSION The incidence rate of the HAP in the elderly patients is high.The long bedridden,indwelling gastric catheter,indwelling urethral catheter,indwelling deep venous catheter,hypoalbuminemia and anemia are the susceptible factors for the HAP.It is of great significance in reducing the incidence of HAP in the elderly patients to control those risk factors.
