社区医院开展家庭病床的SWOT分析与对策研究

摘　　要：目的了解天津市社区医院家庭病床的开展情况,分析实施中存在的问题和影响因素,为开展家庭病床服务提供依据。方法采用目的抽样方法选取18家社区医院,对18位院长进行关于开展家庭病床服务情况的一对一深度访谈。采用SWOT分析方法总结开展家庭病床存在的优势、劣势、机遇及挑战。结果被调查的18家社区医院中,正在开展家庭病床服务的有6家,曾经开展的有9家,从未开展的有3家。受访的18位院长中有9位从事过家庭病床服务,14位表示愿意开展家庭病床服务。影响家庭病床开展的主要因素为:人力资源紧缺,政策制度不完善,医疗技术缺乏,医疗风险高等。结论需规范家庭病床服务管理制度,加强政府政策指导,加快培养全科医生,合理利用医院软硬件条件以及提高社会各界的认知度等。Objective To learn the conditions of health care at home in community hospitals in Tianjin, and analyze the existing problems in implementation and the impacting factors so as to provide the basis for development of the service of sick-bed at home. Methods The purposive sampling was taken to choose eighteen hospital presidents of 18 community hospitals for a deep interview of development of the health care service at home. SWOT analysis was used to summarize the strengths, weaknesses, opportunities and threats in providing the service of sickbed at home. Results Among the 18 investigated hospitals six have provided the service of sickbed at home, nine had ever provided the service before, and three never had. Nine of the presidents had worked on health care service at home and fourteen are willing to do the work. The main factors impacting development of the service at home were limitation of human resource, faultiness of strategies and measures, shortages of medical technologies and high medical risk. Conclusion It is necessary to develop and standardize the administration rules for the service of sickbed at home, enhance the guidance of policies, speed up the training on general practitioners, and reasonably utilize the facilities of both software and hardware so as to raise the cognition degree from society.
