南昌市老年人社区护理服务提供现状调查研究

摘　　要：目的了解南昌市社区卫生服务机构提供老年人社区护理服务的现状,为南昌市卫生行政部门制定合理的规划提供依据。方法采用简单随机抽样的方法,抽取南昌市五大主城区社区卫生服务机构的社区护士(包括中心和站),采用自制的老年人社区护理服务提供现状调查表(内部一致性Cronbach'α值为0.89,内容效度为0.90)进行现场调查。结果社区护士认为经常提供的社区护理服务前三项分别为定期健康体检(占75.6%,152/201),门诊一般治疗护理(占67.7%,136/201)和营养饮食指导(占62.7%,126/201)、从未提供的前三项分别是家庭病床服务(占73.1%,147/201)、心理咨询(占56.7%,114/201)和免疫接种护理指导(占51.7%,104/201);提供质量较好的是定期健康体检(占73.7%,137/186)、健康咨询(占66.1%,119/180)和门诊一般治疗(占65.7%,115/175),提供质量稍差的是家庭病床服务(占13.0%,7/54)、康复护理(占5.4%,8/149)和家庭访视(占5.0%,5/99)。结论目前南昌市老年人社区护理提供现状能在一定程度上满足老年人日益增长的社区护理需求,但是需要在各个方面拓展老年人社区护理服务内容,落实好已开展的服务内容,提高服务的质量,以保证老年人在社区就能享受到社区卫生服务机构提供的健康服务。Objective To understand the present situation of nursing service for community elderlies provided by community health service organizations in Nanchang city in order to provide a basis for the municipal health administration to develop a reasonable plan. Methods The simple random cluster sampling was used to recruit curses for community health service organi- zations of the five main urban zones in Nanchang. A self - designed questionnaire on status of nursing service for community elderlies （ internal consistency Cronbach＇ alpha value is 0. 89, content validity is 0. 90 ） was taken for survey. Results The first three items in community nursing services provided regularly by the nurses were regular physical examination, general outpatient nursing and nutrition diet guidance, accounting respectively for 75.6% （ 152/201 ）, 67.7% （ 136/201 ）, and 62.7% （ 126/ 201 ） . The first three items never provided were family sick - bed service, psychological advisory and immune vaccination nurs- ing guidance, accounting respectively for 73.1% （ 147/201 ） , 56. 7% （ 114/201 ） , and 51.7% （ 104/201 ） . The provided i- tems with better quality were regular physical examination, health advisory and general outpatient nursing, accounting respective- ly for 73.7% （ 137/186 ）, 66. 1% （ 119/180）, and 65.7% （ 115/175 ） . The provided items with slightly poor qiality were family sick -bed service, rehabilitation nursing and follow -up as home, accounting respectively for 13.0% （7/54）, 5.4% （8/149）, and 5.0% （5/99） . Conclusion The currently provided community nursing care for elderlies of Nanchang city can meet the growing demand to a certain extent. Nevertheless, it is necessary to expand the nursing contents for the elderly in various respects, carry out the already provided service completely, and improve the service quality, so as to ensure the elderly to access to the community health services in their communities.
