摘  要:目的探讨HC视频喉镜引导双腔导管气管插管的效果。方法将70例胸科手术患者随机分为2组:HC视频喉镜组(H组,n=35)和Macintosh喉镜组(M组,n=35)。诱导后分别用两种喉镜引导经口插管,对两组患者声门暴露级别、插管时间和血流动力学指标进行分析。结果H组声门暴露满意率(97.1%)高于M组(60.0%,P〈0.05),H组插管总时间[(60.71±8.33)s长于M组(49.47±10.83)S,P〈0.05]。结论与Macintosh喉镜比较,HC视频喉镜引导双腔导管气管插管时间较长,但其能够改善声门暴露程度,可作为胸科困难气道患者的备选插管工具;环状软骨按压可提高两种喉镜引导双腔导管气管插管的成功率。Objective To compare the effects of double-lumen tube placement between HC videolaryngoscope and Macintosh laryngoscope. Methods Seventy ASA I or Ⅱ patients, aged 18-60 years old, with body mass index of 18-24 kg/m2, undergoing elective thoracic surgery, were randomly divided into two groups : HC video-laryngoscope group ( group H, n = 35 ) and Macintosh laryngoscope group ( group M, n = 35 ). The patients of two groups were intubated respectively by two laryngoscopes after induction. The glottal exposed grade, the intubation time and hemodynamic parameters were recorded. Results The glottal exposed satisfaction rate was significantly higher in group H (97.1%) than in group M (60.0%) (P 〈0. 05). The total intubation time was significantly longer in group H [ (60. 71 ± 8. 33 ) s ] than in group M [ (49.47 ± 10. 83) s] (P 〈0. 05). The glottal exposed time and hemodynamic changes between the two groups had no significant difference (P 〉 0. 05). Conclusion HC video-laryngoscope, compared with the Macintosh laryngoscope for double-lumen tube intuhation, can provide better glottic exposure view, but the total intubation time is significantly longer for double-lumen tube intubation using HC video-laryngoscope than Macintosh laryngoscope. Cricoid pressure can improve the intubation success rate of the two laryngoscopes.
