滋阴清热活血方干预糖调节受损患者的临床研究

摘　　要：目的:观察滋阴清热活血方对阴虚内热、气滞血瘀型糖调节受损(IGR)患者的临床疗效。方法:将120例阴虚内热、气滞血瘀型糖调节受损(IGR)患者,随机分为饮食运动组(40例)、西药组(40例)、中药组(40例),3组均进行饮食、运动治疗和糖尿病教育,西药组加用二甲双胍,中药组加用滋阴清热活血方,治疗3个月分别观察3组患者治疗前后血糖、糖化血红蛋白(HbA1c)、胰岛功能、脂代谢、体重指数(BMI)及中医临床症状等的变化。结果:与饮食运动组比较,二甲双胍组和中药组治疗后的空腹血糖(FPG)、OGTT后2h血糖(2hPG)、HbA1c和胰岛素抵抗指数(HOMA-IR)均显著减低(P<0.05);与饮食运动组和二甲双胍组比较,中药组治疗后的临床症状积分有显著减少(P<0.05)。结论:滋阴清热活血方可以显著改善患者血糖及胰岛素抵抗,疗效与二甲双胍相当;在临床症状改善方面优于二甲双胍。Objective：To observe the curative effect of Ziyin Qingre Huoxue Prescription on impaired glucose regulation（IGR）of Yin Deficiency Inner Heat and Qi stagnation and blood stasis.Methods：The patients of IGR of Yin Deficiency Inner Heat and Qi stagnation and blood stasis（n=120）were randomly divided into Diet plus physical exercise intervention group（n=40）,Western medicinal group（n=40）and Chinese medicinal group（n=40）,and was respectively given the basic treatment,basic treatment and Metformin,basic treatment and Ziyin Qingre Huoxue Prescription.The changes of plasma glucose,glycated hemoglobin（HbA1c）,insulin function,lipid metabolism,body mass index（BMI）and the Chinese traditional clinical symptoms were observed.Results：Compared with the Diet plus physical exercise intervention group,the levels of fasting plasma glucose（FPG）and 2-hour postprandial blood glucose（PBG）and HbA1c and insulin resistance index（HOMA-IR）decreased significantly（P0.05）in the medicinal Chinese group and Western medicinal group;Compared with the Diet plus physical exercise intervention group and Western medicinal group the score of traditional Chinese clinical symptoms decreased significantly（P0.05）in the Chinese medicinal group.Conclusion：Ziyin Qingre Huoxue Prescription can relieve glucose and insulin resistance index and the curative effect is as the same as Metformin.The improvement of traditional Chinese clinical symptoms is much better than Metformin.
