PCT和BNP指导下的有创与无创序贯机械通气治疗慢性阻塞性肺疾病急性加重合并肺性脑病的临床研究

摘　　要：目的:通过调查研究在PCT和BNP的指导下,观察有创和无创序贯机械通气对慢性阻塞性肺疾病急性加重(AECOPD)合并肺性脑病的临床效果。方法:采用随机数字表法将68例老年AECOPD合并肺性脑病患者分成有创—无创序贯机械通气组(治疗组)和有创机械通气组(对照组)。结果:治疗组与对照组相比,其有创通气时间与总机械的通气时间明显减少(P<0.05),产生VAP的频率以及住ICU的时间均明显降低(P<0.05)。结论:在PCT和BNP的指导下可以更好寻找时间窗,更加准确掌握拔管时机,成功脱机。有创和无创序贯机械通气对合并肺性脑病的治疗具有安全性与稳定性,值得在临床上进一步推广运用。Objective Through the investigation and study in under the guidance of PCT and BNP, observe how gen and noninvasive sequential mechanical ventilation in AECOPD with pulmonary encephalopathy clinical effect.Methods Random number table method will be 68 elderly cases AECOPD with pulmonary encephalopathy patients into a ＂nothing＂ sequential mechanical ventilation group（treatment group） and mechanical ventilation and group（control group）.Results The treatment group compared with the control,the ventilation and time and total mechanical aeration time significantly reduced（P 〈 0.05 ） ,the frequency of VAP produce and live the time of ICU were significantly lower（P 〈 0.05）.Conclusion Can better find time window in the PCT and BNP＇s guidance,more accurately grasp the extubafion time, successfully weaning. Invasive and noninvasive treatment of sequential mechanical ventilation on pulmonary encephalopathy with security and stability, it is worth popularizing and applying in clinic.
