摘　　要：目的探讨循环CD133+、Toll样受体2(TLR2)和Toll样受体4(TLR4)表达水平对老年陈旧性心肌梗死患者细胞心肌成形术早期预后的影响。方法采用流式细胞仪检测经皮冠状动脉介入治疗(PCI)+支架植入术患者和PCI+支架植入术+人脐带造血干细胞(hUCM-SCs)移植术患者循环CD133+、TLR2和TLR4的表达水平与心肌梗死面积和左心室射血分数(LVEF)的关系。评估CD133+、TLR2和TLR4对细胞心肌成形术早期预后的影响。结果 PCI+支架植入术后8周CD133+(0.05±0.02)%、TLR2(5.9±2.2)荧光强度(MFI),TLR4(3.7±1.8)MFI;PCI+支架植入术+hUCM-SCs移植后8周CD133+(0.07±0.01)%,TLR2(3.6±0.3)MFI,TLR4(2.2±1.3)MFI。2组患者治疗8周后CD133+、TLR2和TLR4水平比较均有显著性差异(P均<0.01)。结论 CD133+、TLR2和TLR4表达水平可能影响老年陈旧性心肌梗死患者细胞心肌成形术的早期预后。Objective It is to observe the influence of the expressions of peripheral blood CD133^＋ , TLR2, TLR4 and early prognosis of cellular cardiomyoplasty in patients with old myocardial infarction. Methods The patients were treated with percutaneous coronary intervention （PCI） and coronary artery stent implantation plus or not human umbilical cord blood stem cells transplantation respectively. The expressions of circulating CD133^＋ , TLR2 and TLR4 were measured by flow cytometry, and the relation with myocardial infarcted area and left ventricular ejection fraction （LVEF） were observed. The influences of the levels of CD133^＋ , TLR2 and TLR4 on prognosis of cellular cardiomyoplasty were evaluatied. Results At eight weeks, the result of CD133^＋ , TLR2 and TLR4 was （0.07 ± 0.01 ） % , （5.9 ± 2.2 ） MFI and （3.7 ± 1.8 ） MFI in patients after cellular cardio- myoplasty, and was （0.05 ± 0.02） % , （ 3.6 ± 0.3 ） MFI and （2.2 ± 1.3 ） MFI in patients after coronary stent implantation respectively. There were significant differences between the two groups （ all P 〈 0.01 ）. Conclusion The levels of CD133^＋ , TLR2 and TLR4 may influence early prognosis of cellular cardiomyoplasty in patients with old myocardial infarction.
