甲基苯丙胺依赖者认知功能与事件相关电位P300的研究

摘　　要：目的探讨甲基苯丙胺依赖者认知功能状况及事件相关电位P300特征。方法随机将年龄在18～65岁之间符合CCMD-3甲基苯丙胺依赖诊断标准患者30例作为实验组,选择30例健康者作为对照组。各组分别予听觉P300检测和蒙特利尔认知评估量表筛查实验,将两组结果加以分析、比较。结果与对照组相比,实验组CZ点P2、N2、P3及PZ点N1、P2、N2、P3潜伏期明显延长(P<0.05或P<0.01);实验组CZ、PZ点波幅明显降低(P<0.05或P<0.01);实验组蒙特利尔认知评估量表7个因子分及总分值均低于对照组,差异有统计学意义(P<0.05或P<0.01);PZ点潜伏期及波幅与蒙特利尔认知评估量表中7个因子分具有相关性:为PZ点N1、P2、N2、P3潜伏期与相关因子及总分呈负相关(P<0.05或P<0.01);PZ点波幅则与各因子分及总分呈正相关(P<0.05或P<0.01)。结论甲基苯丙胺依赖者存在认知功能障碍。P300电位可作为评价甲基苯丙胺依赖者认知状况的客观指标之一应用于临床。Objective To investigate the impairment of cognitive function and the feature of event related potential P300 （ERP 17300） in patients with Methamphetamine dependence. Methods 30 patients aged 18 to 65 years old with Methamphetamine dependence （study group） and 30 well-matched healthy individuals （control group） were assessed with Montreal cognitive assessment （MoCA） and ERP P300. The performance of MoCA and the parameters of ERP P300 were compared between the two groups. Results The latencies of P2, N2, P3 in Cz, as well as the latencies of N1, N2, P2, P3 in Pz were significantly longer in study group than those in control group （P 〈0.05 or P 〈0.01 ）. The amplitudes of Cz and Pz were significantly lower in study group than those in control group （ P 〈 0.05 or P 〈 0.01 ）. Total score and 7 factor scores of MoCA in study group were significantly lower than those in control group （P 〈 0.05 or P 〈0.01 ）. In study group, the latencies of N1 , N2, P2, P3 in Pz were significantly negatively correlated with total score and factor scores of MoCA （P 〈0.05 or P 〈0.01 ）, the amplitude of Pz were significantly positively correlated with total score and factor scores of MoCA （P 〈 0.05 or P 〈 0.01 ）. Conclusion The cognitive function is impaired in patients with methamphetamine dependence. Event-related potential P300 can be used as an objective index to assess the impairment of cognitive function in patients with Methamphetamine dependence.
