4～8℃生理盐水在关节镜术后冲洗中的应用效果观察

摘　　要：目的:探讨4～8℃生理盐水在关节镜术后冲洗中的应用效果。方法:将34例膝骨性关节炎患者随机分为持续冲洗组和非冲洗组各17例,冲洗组在行关节镜手术治疗后给予4～8℃生理盐水3 000 ml持续冲洗关节腔,6 h内冲洗完毕;非冲洗组行关节镜手术治疗后不进行关节腔冲洗。术前、术后1 d、7 d分别采用视觉模拟评分(VAS)评定、测量膝关节周径;术前及术后1周、2周根据JOA标准对患膝关节功能状况进行评分。结果:术后1 d、7 d冲洗组疼痛程度、肿胀程度低于非冲洗组(P<0.05);术后1周、2周冲洗组JOA评分高于非冲洗组(P<0.05)。结论:关节镜术后用4～8℃生理盐水持续冲洗关节腔6 h,具有镇痛、消肿、促进膝关节功能恢复、提高关节镜手术疗效的作用。Objective：To investigate the effect of 4～8 ℃ normal saline in joint cavity flushing after arthroscopic surgery. Methods：34 patients with knee osteoarthritis were randomly divided into flushing group and non - flushing group （ 17 cases in each group ）. The patients in the flushing group were given continuous knee joint cavity flushing with 3000 ml of 4～8℃normal saline after arthroscopic operation and the flushing lasted 6 hours;The flushing was not given to the patients in the non -flushing group after arthroscopic operation. The knee circumference was respectively assessed and measured by visual analogue scale （ VAS } before operation,one day and seven days after operation; the scores of knee joint function were marked according to the JOA standard before operation, one week and 2 weeks after operation. Results ： The pain and swelling degree was lower in the flushing group than the non - flushing group in one day and 7 days after operation （ P 〈 0.05 ） ; the JOA scores were higher in the flushing group than the non - flushing group in one week and 2 weeks after operation （P 〈 0.05 ）. Conclusion：The continuous joint cavity flushing with 4～8℃ normal saline for 6 hours has the effeet of easing pain, relieving swelling,promoting recovery of knee joint function after arthroscopic operation,which can improve the efficacy of arthroseopic operation.
