经鼻内镜选择性鼻中隔黏膜下切除术

摘　　要：目的探讨经鼻内镜选择性鼻中隔黏膜下切除的手术方法及意义。方法根据鼻中隔偏曲的类型和程度,对657例鼻中隔偏曲患者采用经鼻内镜选择性鼻中隔黏膜下切除术,其中单纯鼻中隔偏曲145例,合并有鼻息肉、鼻窦炎67例,合并下鼻甲代偿性肥大445例均同期行鼻内镜手术处理。结果 657例中术后随访3个月,失访12例。治愈605例(92.1%),好转40例(6.1%),12例失访(1.8%),1例术后早期鼻中隔血肿,经放置引流皮片处理后治愈;2例发生中鼻甲和中隔黏膜粘连,1例发生下鼻甲和中隔黏膜粘连,均经鼻内镜下分离而好转;1例发生鼻中隔后下方小穿孔,后4例均合并有鼻窦炎鼻息肉或下鼻甲肥大。结论根据鼻中隔偏曲的类型和程度不同,经鼻内镜选择性鼻中隔黏膜下切除术创伤小、安全,效果确切。Objective To probe the operative methods of selective submucous resection of nasal septum using nasal endoscope and its clinical significance. Methods According to the type and degree of deflection of nasal septum, selective submucous resection of nasal septum using nasal endoscope was performed on 657 patients. In 657 cases, 145 had simple scptal deviation, 67 had septal deviation with sinusitis and polyps, 445 had deviated nasal septum with inferior turbinate hypertrophy. Results Follow-up 3 months, 605 cases were cured, 40 were improved and 12 were withdrawn. 1 case with hematoma of nasal septtnn was healed after setting elastic drainage, 2 cases of nasal adhesion were healed by severing the nasal adhesion under endoscope. 1 case of small perforation located below and posterior of nasal septum. These 4 cases all complexly had sinusitis, nasal polyp or turbinate hypertrophy. Conclusion The selective nasal septum reconstruction depending on type and degree of deflection of nasal selptum using nasal endoscope is a minimally invasive,safety and effective method.
