补肾健脾颗粒对肾虚型慢性HBV携带者HBeAg、HBV DNA和肝组织学的影响

摘　　要：[目的]观察补肾健脾颗粒对肾虚型E抗原阳性Hepatitis B e-antigen.(HBeAg)阳性慢性乙肝病毒(Hepatitis B Virus,HBV)携带者乙肝病毒脱氧核糖核酸(HBV deoryiboncleicacid,HBV-DNA)、HBeAg及对肝组织病理的影响。[方法]筛选肾虚型HBeAg阳性慢性HBV携带者60例,随机分为治疗组和对照组两组。治疗组患者给予补肾健脾颗粒治疗,对照组给予安慰剂治疗,共48周。用药前及治疗结束时测定肝功能、乙肝标志物、HBV-DNA、肝脏组织病理等。[结果]治疗结束后,治疗组HBeAg血清学转换率(5.00%)及血清HBV-DNA阴转率(7.50%)显著高于对照组(P<0.05);治疗组治疗前后炎症活动分级及纤维化分期相比显著差异(P<0.01);而对照组在治疗前后无明显差异;临床观察显示补肾健脾颗粒有较好的安全性。[结论]补肾健脾颗粒有利于肾虚型HBeAg阳性慢性HBV携带者HBeAg阴转和HBV-DNA阴转,对肝脏组织炎症活动程度及纤维化分期有改善作用,安全性较好。[Objective] To explore the effect of ＂Bushen Jianpi Capsule＂ on HBV DNA,HBeAg and liver histology of chronic hepatitis B virus carriers with kidney deficiency syndrome. [Methods] 60 cases of HBeAg positive chronic HBV carriers with kidney deficiency syndrome were randonaly divided into two groups with the proportion of 2：1, 40 cases and 20 cases in the treatment group and the control group, respectively. The treatment group was treated with Bushen Jianpi Capsule, the control group was given placebo, the treatment period was 48 weeks. Before and after treatment, Hepatitis B virus markers, HBV-DNA level and hepatic pathology were tested. [Result] HBeAg seroconversion rate and the negative conversion rate of serum HBV-DNA in the treatment group were significantly higher than those of the control group; The inflammatory activity grade and fibrosis stage in the treatment group got better after treatment than that before treatment; while the control group had no significant difference; clinical observation showed better security. [Conclusion] Bushen Jianpi Capsule might improve HBeAg seroconversion rate and the negative conversion rate of serum HBV-DNA, the liver tissue inflammation and degree of fibrosis stages with better security in the HBeAg positive chronic HBV carriers with kidney deficiency syndrome.
