主动脉瓣瓣叶、窦部及瓣叶结合点到窦部的三维径线改变对轻度主动脉瓣反流影响的超声研究

摘　　要：目的探讨主动脉瓣瓣叶、窦部(diameter of the sinus,Ds)及瓣叶结合点到窦部的三维径线改变在轻度主动脉瓣反流(aortic regurgitation,AR)机制中的作用。方法将反流量容积(reverse flow volume,RFV)<30mL纳入轻度AR组(68例),将无反流者纳入对照组(42例),分别行经食管实时三维超声心动图(real timethree-dimensional transesophageal echocardiography,RT-3D-TEE)检查,比较两组间主动脉瓣瓣叶、窦部及瓣叶结合点到窦部的三维数据的差异。结果 (1)轻度AR组仅Ds增大;对照组和轻度AR组均为右冠瓣最长、左冠瓣次之、无冠瓣最短。(2)轻度AR组仅Ds进入Logistic回归方程:Y=6.127+4.236 XDs。(3)Ds在ROC曲线下面积最大。结论三维视野下,窦部内径的增宽与轻度AR有关,瓣叶长度和瓣叶结合点到窦部的距离在轻度AR中无明显改变。Objective To explore the mechanism of AR in the change of aortic valve leafage, diameter of the sinus （Ds） and sinus to the sinus tube junction in patients with mild aortic regurgitation （AR）. Methods Cases whose reverse flow volume （RFV） were less than 30 mL were in mild AR group, and whose RFV were none were in control group. 42 patients in control group and 68 patients in mild AR group was quan tilled by real-time three-dimensional transesophageal echocardiography （RT-3D-TEE） and all aortic valve leafage, sinus and sinus to the sinus tube junction parameters were compared between groups. Results （1） Only Ds was increased in mild AR group. Length of closed lines of the right corollalobes was the longest, length of closed lines of the left corollalobes was longer and lengths of closed lines of the non corol- lalobes was the shortest in both groups. （2） Only Ds in mild AR group was checked in Logistic regutitati on： Y ： 6. 127＋4. 236X Ds. （3）Ds was the widest in ROC curves. Conclusion The increased of the Ds is correlated with mild AR, while there are no significant difference in lengths of closed lines of aortic valve leafage and the diameter of sinus to the sinus tube junction under the three dimensional imaging.
