摘  要:目的探讨窦性心率震荡(heart rate turbulence,HRT)对急性心肌梗死(acute myocardial infarction,AMI)的预测价值。方法选取2008年10月~2011年9月我院心内科确诊为AMI的住院患者60例为AMI组,以同期在我院住院的存在室性前期收缩的非AMI患者60例为对照组,监测24 h动态心电图,观察HRT的两个指标震荡初始(TO)和震荡斜率(TS)的变化,并对AMI组患者作为期10~24个月的随访。结果 AMI组的TO、TS结果分别是(-0.51±1.73)%、(6.03±4.50)ms/RR间期,与对照组TO、TS比较,AMI组TO明显升高、TS明显降低,差异均有统计学意义(P<0.05),对AMI组患者作了10~24个月的跟踪随访,期间死亡11例,存活49例。存活49例的TO、TS值分别为(-0.62±1.57)%、(5.78±3.30)ms/RR间期,与死亡11例比较,存活组TO明显降低、TS明显升高,差异均有统计学意义(P<0.05)。结论 HRT的减弱或消失可作为AMI患者预后的预测指标。Objective To investigate the predictive value of sinus heart rate turbulence (HRT) for acute myocardial infarction(AMI). Methods Sixty cases of hospitalization patients were diagnosed the AMI in cardiology department of our hospital from October 2008 to September 2011 as the AMI group. 60 cases of non-AMI patients were selected as the control group. 24 h ambulatory ECG was monitored. The two indicators of turbulence onset (TO) and turbulence slope (TS) changes were observed. Results The results of TO and TS in AMI group were (-0.51±1.73)% and (6.03±4.50) ms/RR interval. Compared to control group, the value of TO in AMI group was significantly increased, the value of TS was significantly reduced (P 〈 0.05). Compared to death group, the value of TO in survival group was significantly reduced, the value of TS was significantly increased (P 〈 0.05). Conclusion The diminished or disappeared of HRT as the prognosis of patients with AMI predictor.
