联合检测CTnI、hs-CRP、NT-ProBNP对心力衰竭的诊断及预后评估

摘　　要：目的探讨CTnI、hs-CRP、NT-ProBNP联合检测对心力衰竭(HF)的诊断及预后评估。方法 138例HF患者于入院时、治疗第5天、出院时测定血清CTnI、hs-CRP、NT-ProBNP,随访1年观察患者心脏事件的再发生情况,另选同期健康体检者49例作为对照组。结果入院时HF患者CTnI、hs-CRP、NT-ProBNP水平明显高于对照组,心功能越差,其浓度越高(P<0.05);治疗第5天、出院时各指标明显降低(P<0.05);三者联合检测的灵敏度、特异度分别是95.7%和86.0%,明显优于单项检测(P<0.05);出院后随访1年,发生心脏事件者36例,发生心脏事件的患者CTnI、hs-CRP、NT-ProBNP水平明显高于未发生心脏事件患者的水平(P<0.05)。结论血清中CT-nI、hs-CRP及NT-ProBNP在HF患者中明显升高,三者的联合检测对HF临床诊断和预后判断具有重要意义。Objective To discuss the value of diagnosis and prognosis diagnostic on heart failure patients by joint detection with CTnI, hs-CRP and NT-ProBNP. Methods A total of 138 cases of HF patients were measured the serum CTnI, hs-CRP, NT-ProBNP at different time. 49 cases of healthy persons during the same period as a control group to comparative afialysis. Results The CTnI, hs-CRP, NT-ProBNP levels was significantly higher than the control group, and the cardiac function was poorer, the concentration was higher. At the treatment 5 d and discharge, they were all lower than untreated （P 〈 0.05）. The joint detection sensitivity and specificity were 95.7%and 86.0% ,they were all higher than the seprate examination （P 〈 0.05）. After one year, 39 cases had cardiac events, and the CTnI, hs-CRP, NT-ProBNP were all higher than the without recurrence （P 〈 0.05）. Conclusion The serum of CTnI, hs-CRP and NT-ProBNP are all significantly higher in patients with HF, the combined detection of three has important clinical diagnosis and prognosis in the HF patients.
