心理干预在基层医院初孕妇女无痛人流中的应用

摘　　要：目的探讨心理干预在基层医院用于无痛人工流产术护理中的临床效果。方法 100例自愿接受无痛人工流产的孕妇随机分为两组,对照组按无痛人流护理常规进行治疗及护理,干预组在此基础上采用心理干预的护理方法。观察记录两组患者手术出血量、平均手术时间、患者术后呕吐发生率、宫缩痛程度,记录两组患者总体满意度及术前和术后患者的焦虑和抑郁评分。结果两组术后焦虑和抑郁评分比较,干预组低于对照组,差异有统计学意义(P<0.05)。干预组患者满意度明显高于对照组,差异有统计学意义(P<0.05);两组患者手术时间、术中出血量、术后呕吐发生率及宫缩痛比较,差异无统计学意义(P>0.05)。结论心理干预的方法及措施列入人工流产护理计划中,改善了患者的抑郁和焦虑状态,明显提高了满意度并建立了信任的护患关系。Objective To investigate clinical effects of psychological intervention for induced abortion women in primary hospital care. Methods A total of 100 pregnant women who accepted voluntarily painless abortion were randomly divided into two groups. The control group were treated and cared under the care routine of painless abortion. The intervention group accepted psychological intervention on the base of care routine. Observed and recorded two groups of patients about surgical blood loss, mean operative time, postoperative vomiting incidence, degree of uterine contraction pain, overall satisfaction, anxiety and depression scores about preoperative and postoperative patients. Results The postoperative anxiety and depression scores in the intervention group were lower than the control group,the difference was statistically significant （P 〈 0.05 ）. The patient satisfaction in the intervention group was significantly higher than the control group and the difference was statistically significant （P 〈 0.05）. The operative time, blood loss, and postoperative vomiting incidence,uterine contraction pain were not statistically significant between two groups of patients （P 〉 0.05）. Conclusion The ways and measures of psychological intervention involve in the artificial abortion care plan, which improve the state of depression and anxiety of patients, can significantly improve the satisfaction and establish a trust relationship between nurses and patients.
