腹腔镜手术中右旋美托咪啶对全凭静脉麻醉的影响

摘　　要：目的探讨腹腔镜手术全身麻醉中全程复合右美托咪啶对全凭静脉麻醉的影响。方法 50例患者随机分为右旋美托咪啶组(D组)和对照组(C组)。观察注药前(T1)、麻醉诱导前(T2)、气管插管前(T3)、气管插管即刻(T4)、气腹时(T5)、气腹后每15 min(T6、T7、T8、T9)、手术结束时(T10)、拔管即刻(T11)、拔管后5 min(T12)的HR、SBP、DBP的变化。结果 D组患者HR、SBP和DBP在T2、T3、T4、T5、T11低于C组(P<0.05),并在T2-T11均低于T1(P<0.05);C组患者HR、SBP、DBP在气管拔管时增高(P<0.05)。结论腹腔镜手术全凭静脉麻醉复合右旋美托咪啶血流动力学指标更平稳。Objective To evaluate whether dexmedetomine （Dex） has an effect on total intravenous anesthesia undergoing laparoscopic surgery. Methods Fifty patients were randomized to either a dexmedetomidine group （group D） or the control group （group C）. SDP, SBP, HR were recored at the time point of pe-administation of Dex （T1）, before induction （T2）, Intubation （T3）, at pneumoperitoneum （T4）, then every 15 min （T5, T6, T7, T8, T9）, end of surgery（T10）,extubation （T11）, 5rain after extubation （T12）. Results In the group D, HR, SBP and DBP was decrease at T2, T3, T4, T5 and Tll compared to group C （P 〈 0.05） , decreased at T2～T11 compared to T1 （P〈 0.05）. In the group C, HR, SBP and DBP had a increase at extubation （P〈 0.05）. Conclusion Dexmedetomidine is an effective anesthetic adjuvant that can be safely used in laparoseopy with hemodynamic indexes more smoothly.
