WHO生存质量简表评价脑卒中患者信度及效度研究

摘　　要：目的评估世界卫生组织生命质量测定量表简表（WHOQOL—BREF）应用于脑卒中患者的信度和效度。方法采用自填法结合访谈法，选取2010年1月1日至2010年3月31日在天津中医药大学第一附属医院针灸门诊以及天津市医华针灸门诊就医的100例脑卒中患者，用WHOQOL—BREF评定生活质量。信度评价采用内部一致性分析（Cronbach’α系数）、分半信度（Guttman系数），并对所有的条目进行深入分析，筛选出区分度不高的条目。效度用相关分析法评价内容效度和区分效度，用因子分析法评价结构效度。结果有效问卷83份。信度：Cronbach’α／系数为0．805，Guttman系数为0．731。效度：（1）量表除Q25外各条目与总分的相关系数绝对值在0．398～0．745；各领域与其所包含条目的相关系数绝对值在0．295～0．826；均具有统计学意义。（2）各维度与其它维度间的相关系数均小于该维度与总分问的相关系数。（3）根据特征根大小共筛选8个公因子，累计贡献率为74．726％。结论WHOQOL—BREF在脑卒中患者中的应用信、效度表现良好，结构效度与原量表的结构发生变化。Objective To assess the reliability and validity of the World Health Organization Quality of Life -BREF （WHOQOL-BREF） in patients with stroke. Methods From January 1,2010 to March 31, 2010, a total of 100 stroke patients who visited First Teaching Hospital of Tianjin University of Traditional Chinese Medicine Acupuncture Clinic and Tianjin Yihua Acupuncture Clinic were collected and the life qualities of the patients were evaluated with WHOQOL-BREF. Reliability （internal consistency and Guttman split-half） and validity （content validity, discriminant validity and construct validity） were tested by means of statistical. Results A total of 83 valid responses were collected. Reliability analysis of WHOQOL-BREF showed that the Cronbach＇s a and split-half reliability coefficients were 0.805 and 0.731 respectively. According to validi- ty analysis of WHOQOL-BREF, （1）there was significant correlation among the scores of most questions and the total score was 0.398-0.745. The correlation coefficient between that of the questions and it＇s domains were 0.295-0.826; （2）the correlation coefficients between individual domain score and other domain scores were greater than this domain score and the total score; （3）through the factor analysis, eight common factors were worked out and the cumulative contribution rate was 74.726%.Conclusion WHOQOL-BREF scale has good reliability and performance validity and the structural validity has changed.
