HANS超前镇痛作用的临床观察及对血清IL-6的影响

摘　　要：目的 :观察预先应用韩氏穴位神经刺激仪 (HANS)的超前镇痛作用及对血清白细胞介素 6(IL 6 )的影响。方法 :选择妇科择期手术患者 30例 ,ASA Ⅰ ～ Ⅱ级 ,随机分为两组 ,各 15例 ,两组均在硬膜外麻醉下手术并于术毕硬膜外注入吗啡镇痛 ,实验组应用HANS超前镇痛。观察两组术后镇痛的情况及血清IL 6的变化。结果 :两组硬膜外吗啡术后镇痛时间及静息时的视觉模拟评分 (VAS)差异无显著性 ,实验组运动时的VAS低于对照组 (P <0 0 5) ,加用其它镇痛药的次数也少于对照组 (P <0 0 5) ;与术前相比 ,两组术后血清IL 6水平均升高 ,但实验组升高时间较对照组短 (P <0 0 5)。结论 :预先应用HANS具有超前镇痛作用 ,并对血清ILObjective:To observe the effect of pre emptive analgesia with Han's Acupoint and Nerve Stimulator(HANS) on postoperative pain and change of serum IL 6. Methods:30 patients(ASAⅠ～Ⅰgrade)undergoing elective pelvic surgery were randomly divided into two groups(n=15).All patients received epidural anesthesia and postoperative analgesia with epidural morphine,HANS group was also received preoperativee HANS. Results:There was no significant difference between two groups for the duration of postoperative analgesia with epidural morphine and visual analogus scale(VAS) at rest,but the VAS at movement was lower and additional analgesic requirement was less in HANS group(P<0 05).Serum IL 6 level increased in both groups after operation,but in HANS group it was lower on the third day than on the first day after operation (P<0.05). Conclusion:Preoperative treatment with HANS has the effect of pre emptive analgesia and can reduce the change of serum IL 6.
