摘 要:目的探讨白细胞滤除对红细胞功能的影响。方法将30名健康献血员捐献的血液制备成悬浮红细胞,随机分为实验组[白细胞滤除组,使用BC.L400型去白细胞滤器(成都双流公司)]和对照组,常规保存。分别在第0、7、14、21、28、35天进行红细胞的血常规、Na+、K+、乳酸(LD)、葡萄糖(Glu)、血气、血流变、Na+-K+-ATP酶、游离血红蛋白(FHb)等指标检测。结果红细胞悬液滤过后其中的白细胞显著减少(P<0.01);随着保存时间的延长,两组红细胞的血常规、Na+、K+、LD、Glu各项指标间差异无统计学意义(P>0.05);两组FHb含量明显增加,且在保存28d时实验组明显高于对照组(P<0.01);保存0~14d时两组红细胞内Na+-K+-ATP酶逐渐减少,实验组显著高于对照组(P%<0.01);血气指标中保存液pH值随保存时间延长而下降,保存第35天时实验组高于对照组(P<0.01),氧分压(PO2)、二氧化碳分压(PCO2)、血氧饱和度为50%时的氧分压(P50)等检测结果两组间差异无统计学意义(P>0.05);两组红细胞高切黏度和低切黏度值均随保存时间延长而不断升高,21d时两组间比较差异有统计学意义(P<0.01)。结论此种国产白细胞滤器滤除白细胞可能对维持体外保存红细胞的理化性质和功能有一定影响。Objective To evaluate the clinical effectiveness of leukocyte-depleted red blood cells (RBC) by study- ing the effect of leukocytes removal on the physiological and biochemical properties and function of RBC during storage. Methods Thirty bags of red blood eel1 suspension were prepared from the 400 ml whole blood of 30 healthy donors. Each bag of RBCs were divided into two parts. The blood were divided into the control group (without leukocytes removal) and the test group (with leukocytes removal). The blood routine test, venous blood gas analysis, hemorhedogic properties, activity levels of Na+-K+-ATP enzyme, concentrations of Na+ ,K+,LD.Glu of the two groups were measured. Results WBC counting decreased significantly after leukocytes removal (P 〈 0.01). There was no significant difference of Na+,K+,Glucose concentrations between the two groups (P 〉 0.05). During the storage of 0N14 d, the activity levels of Na+-K+-ATP enzyme in the two groups were decreased and the activity levels of Na+-K+-ATP enzyme of the test group were significantly higher than those of the control group (P 〈 0.01). There was no significant difference in blood gas analysis including PO2,PCO2, PS0 between the two groups (P 〉 0.05), except the pH, which decreased with storage duration, and the pH was higher in the test group than that of the control group (P 〈 0.01). The high and low shear rates were raised remarkablely after 21 d, and the difference between the two groups was significant (P 〈 0.01). Conclusion Leukocytes removal is beneficial to RBC preservation in vitro storage.
