摘　　要：目的 :探讨影响 Steffee手术治疗胸腰椎骨折疗效的原因。方法 :通过对 46例术前术后 X线片进行 Cobb's角、钉杆角及 X线片和 CT片椎管矢径的测量。结果 :38例椎体后凸 Cobb's角矫正良好。全部病人经手术固定椎管容积增大 ,骨折脱位复位满意。结论 :螺钉的位置和钉杆角是椎体复位的关键 ,α角 + β角≥ 180°对后凸畸形 Cobb角矫正明显 (P <0 .0 1)。对骨折椎体前柱的复位单靠器械杠杆的力量是不足的 ,还应依靠术中俯卧位胸腰椎段背伸牵引力量。Steffee手术固定可使椎管容积增大。Objective:To analyse the influencing factors of Steffee's operation result in the treatment of thoraco-lumbar fracture Methods:we measured the Coob's angles and screw Rod angles on X-ray film and the sagittal diameter of vertebral canal on CT film in 46 cases before and after operation Results:In 38 patients(82 6 percent) the anterior column of vertebrae were corrected All patient's space of vertebral canal were enlarged,the vertebral fracture and dislocation were perfectly reduced Conclusion:The results indicated that Screw-Rod angles and the position of screw is the most important factor in reduction α angles+β angles>180° will made a better results in correction for Cobb's angles in kyphosis The correction of the anterior column of vertebra was not enough to hold up the mechanic strength of lever It also mainly rely on the strength produced by the over-stretch of the Thoraco-Lumbar region in the prone position with the use of Steffee's fixator
