血同型半胱氨酸与老年冠心病患者冠脉病变及内皮功能相关性研究

摘　　要：【目的】探讨血同型半胱氨酸（Hcy）与老年冠心病患者冠脉病变及血管内皮功能的相关性。【方法】对109例老年冠心病患者血Hcy进行检测，根据其水平分为正常组（N=49）与高Hey组（n=60），采用冠状动脉造影及评分观察冠脉病变情况；肱动脉超声评估动脉内皮功能状态。【结果】与正常组相比，高Hcy组冠脉损伤评分显著升高，三支病变所占比例显著上升，且均具有统计学意义（P〈0．05）；高Hcy组血流介导的血管舒张功能显著降低，具有统计学意义（P〈0．05）。【结论】高Hcy血症可反映老年冠心病患者冠脉损伤及血管内皮功能下降，可作为临床辅助检查指标。[Objective] To explore the relationship of blood homocysteine（Hcy） with coronary lesions and vascular endothelial function in elderly patients with coronary heart disease. [Methods]Blood Hcy level of 109 elderly patients with coronary heart disease were detected. According to blood Hcy level, the patients were di- vided into high Hcy group（ n -~ 60） and normal Hcy group（ n = 49）. Coronary angiography was applied to ex- amine and assess coronary lesions, and brachial artery ultrasonography was applied to evaluate vascular endo- thelial function. [Results] Compared with control group, coronary artery damage score and the percentage of triple vessels disease in high Hcy group increased markedly, and there were significant differences（ P 〈0. 05）. Vasodilation function mediated by blood flow in high Hey group decreased markedly, and there was significant difference（ P〈0.05）. [Conclusion]Hyperhomocysteinemia can reflect coronary artery injury and dysfunction of vascular endothelium, and can be used as an auxiliary clinical examination.
