摘  要:目的对比分析高龄冠心病患者经桡动脉与股动脉途径冠脉介入治疗的优缺点,评价经桡动脉途径介入治疗高龄冠心病患者的有效性与安全性。方法 2010年1月至2012年6月我院行冠脉介入治疗超过75岁冠心病患者168例,按穿刺途径随机分成桡动脉组(n=88)和股动脉组(n=80),比较两组的穿刺成功率、手术成功率、X线照射时间、造影剂用量、手术并发症和住院时间。结果手术成功率、造影剂用量、X线照射时间及手术时间两组比较差异无统计学意义(P>0.05);但桡动脉组穿刺部位并发症、术后不适发生率及住院时间低于股动脉组,差异有统计学意义(分别为P<0.05和P<0.01)。结论高龄冠心病患者经桡动脉途径行冠脉介入治疗安全有效,并发症少,住院时间短,患者易于接受。Objective To evaluate the efficacy and safety of the transradial approach in percutaneous coro- nary intervention (PCI) of elderly patients with coronary artery disease by contrasting with the transfemoral approach. Methods The randomized controlled study of 168 elderly patients for percutaneous coronary intervention was car- ried out in our hospital from January 2010 to June 2012. These patients were randomly divided into the radial artery group (n=88) and the femoral artery group (n=80). The success rate of puncture, the success rate of surgery, X-ray irra- diation time, the amount of contrast agent, surgical complications and hospitalization time were compared between the two groups. Results The difference in the surgical success rate, the amount of contrast agent, X-ray irradiation time and the operation time between the two groups was not statistically significant (P〉0.05). However, the vascular com- plications, the postoperative discomfort incidence and hospitalization time of the radial artery group were lower than those of the femoral artery group, the difference was statistically significant (P〈0.05 and P〈0.01). Conclusion The transradial approach in percutaneous coronary intervention is safe and effective for treating elderly patients with coro- nary artery disease, with fewer complications, shorter hospital stay, which is easily accepted by patients.
