摘 要:目的观察西地那非治疗新生儿持续肺动脉高压(PPHN)的疗效。方法对2011年4月至2012年12月收住本科常规治疗无效且符合病例纳入标准的30例PPHN患儿加用西地那非口服治疗,剂量为1mg/(kg·次),q6h。采用自身前后对照,对比治疗前后的肺动脉压力(PAP)、血气分析、氧合指数(OI)、平均血压(MAP),观察治疗效果。结果 30例患儿加用西地那非治疗后PAP明显降低(F=6.34,P<0.05),血气分析PaO2、PaCO2、PH明显改善(F=44.67、11.99、14.20,P<0.05),OI升高(F=27.52,P<0.05),MAP变化不明显(F=3.02,P>0.05)。结论西地那非能有效降低肺动脉压力,改善PPHN患儿的氧合功能,对体循环血压无明显影响。Objective To investigate the effects of sildenafil on newborns with persistent pulmonary hypertension (PPHN). Methods From April 2011 to December 2012, 30 newborns with PPHN pretreated with routine treatment were en- rolled and received sildenafil by the dose as 1 mg/(kg, time), q6 h. Pulmonary artery pressure (PAP), blood gas analysis, oxy- genation index (OI), mean artery pressure (MAP) were recorded before and after the treatment. Statistical analysis was used with analysis of variance. Results PAP of 30 newborns decreased significantly after using sildenafil (F=6.34, P 〈 0.05); PaO2, PaCO2, PH, OI were significantly improved (F=44.67,11.99,14.20, 27.52, P 〈 0.05); and the significant change was not found in MAP (F=3.02, P 〉 0.05). Conclusion Sildenafil is effective for the treatment of PPHN by decreasing pulmonary ar- tery pressure and improving oxygenation function, and the effect on systemic blood pressure is moderate.
