不同剂量帕瑞昔布钠预处理减轻异丙酚注射痛的临床观察

摘　　要：目的研究不同剂量帕瑞昔布钠预处理减轻异丙酚注射痛的效果。方法采用随机双肓对照方法，选择我院择期手术患者160例，ASAⅠ-Ⅱ级，随机分为四组，每组40例。麻醉诱导前2min，均从手背静脉预给药。A组：静脉注射帕瑞昔布钠40mg；B组：静脉注射帕瑞昔布钠20rag；均溶于5ml生理盐水；C组：静脉注射利多卡因40mg；D组：静脉注射0．9％生理盐水5ml。给药2min后以0．5ml／s的速度注射异丙酚。注射期间使用4分法对注射痛评分。结果与B组和D组比较，A组和C组异丙酚注射痛发生率明显降低（P〈0．01）。四组患者异丙酚注射痛的VRS评分在轻度疼痛方面差异无统计学意义。但在中至重度疼痛方面A组、B组和C组明显低于D组（P〈0．01）。结论预先2min静脉注射帕瑞昔布钠40mg能明显降低异丙酚注射痛。静脉注射帕瑞昔布钠20mg能降低异丙酚注射痛的程度但不能降低异丙酚注射痛的发生率。Objective To investigate the effect of different doses of parecoxib pretreatment on propofol in- jection pain. Methods A total of 160 ASA patients with physical status Ⅰ-Ⅱ scheduled for elective surgery were randomized to four groups, with 40 patients each, to receive pretreatment with parecoxib 40 mg （Group A）, parecoxib 20 mg （Group B）, lidocaine 40 mg （Group C）, or normal saline （Group D）. All pretreatment drugs were prepared in 5 ml doses. After 2 minute drug injection, the proplfol was administered. Pain scores were obtained by a study-blinded observer during propofol injection following the different pretreatment solutions, Results Pain scores among the four groups were significantly different. Pain was significantly reduced in Group A （P≤0.01） compared with the con- trol group, Moderate to severe pain was experienced by 33 （82.5%） patients in Group D and 4 （8%） patients in Group B, whereas no patient in Group A （P〈0.01）, Reduction in pain severity was statistically significant after pretreatment with either parecoxib 20 mg （P=0.002） or parecoxib 40 mg （P〈0.01）. Conclusion Parecoxib 40 mg pretreatment is effective to reduce the frequency and severity of pain with propofol injection. Pretreatment with 20 mg ofparecoxib re- duces the severity of propofol injection pain significantly but does not reduce freqttency.
