摘 要:【目的】探讨不同针刺介入时间对围产期全脑缺氧缺血性脑损伤(HIBD)新生模型大鼠大脑皮质凋亡细胞、巢蛋白(Nestin)表达的影响。【方法】于SD雌性大鼠妊娠21 d时,颈椎脱臼法处死,用止血钳夹闭双侧子宫角血管5 min,剖宫产取出的幼鼠经行为学及脑组织切片确认为缺氧缺血性脑损伤后,随机分为模型组、针刺组。模型组不针刺;针刺1组、针刺2组、针刺3组、针刺4组分别于出生后第3、5、7、14天开始针刺,连续7 d;正常组自然分娩,不造模不针刺;假手术组为孕鼠被处死后,不钳夹子宫动脉而剖宫取出幼鼠。各组大鼠均于出生后21 d断头取脑组织,检测大脑皮质中凋亡细胞及Nestin的表达。【结果】(1)各针刺组凋亡细胞的表达均有减少;针刺1组与模型组比较差异有统计学意义(P<0.05);(2)各针刺组的Nestin表达均有所增加,针刺1组、针刺2组、针刺3组与模型组比较差异有统计学意义(均P<0.01)。【结论】针刺抗缺氧缺血作用可能是通过减少大脑神经细胞凋亡、增加脑组织中Nestin的表达来实现,出生后第5天开始针刺作用较好。Objective To investigate the effect of acupuncture intervention time on neuron apoptosis and Nestin expression in the cerebral cortex of the neonatal rat model with perinatal whole brain hypoxic-ischemic brain damage (HIBD). Methods SD female rats being pregnant for 21d were sacrificed by cervical dislocation. After the vessels in the bilateral uterine horns were clamped for five minutes, the rats were given cesarean section for the birth of the fetus. The rat fetus model of HIBD was confirmed by behavioral features and brain tissue slices examination. The modeled rats were randomly divided into model group and acupuncture groups 1, 2, 3 and 4 (giving acupuncture on day 3, 5, 7, 14 after birth, for seven consecutive days). Meanwhile, normal group (giving birth by natural childbirth) and sham operation group (only exposing the uterus of pregnant rats but not clamping the uterine vessels after killing the rats) were also established. All the baby rats were executed on day 21 after birth, and then the apoptotic cells and Nestin expression in the cerebral cortex were detected. Results (1) The expression of apoptotic cells in all acupuncture groups showed a decreasing trend, but only the acupuncture group 1 differed from the model group(P〈0.05). (2) The expression of Nestin in acupuncture groups 1, 2, and 3 was increased as compared with the model group, the difference being statistically significant (P〈 0.01). Conclusion The mechanism of acupuncture for counteracting hypoxia and ischemia may be related with reducing apoptosis and promoting the expression of Nestin in the I-IIBD rats, and acupuncture intervention from day 5 after birth is more effective.
