彩色多普勒超声评估中药洗浴对肱动脉血流的影响

摘　　要：目的:探讨中药熏洗对痉挛型脑瘫患儿肱动脉血流动学状态是否存在改变及确定最佳洗浴温度。方法:对本院120例次痉挛型脑瘫患儿在中药洗浴及水疗前后用彩色多普勒超声观察其肱动脉的血流平均速度(MV),收缩期峰值速度(PSV),舒张末期速度(EDV),收缩早期加速时间(AT),阻力指数(RI),搏动指数(PI),与正常组作对照,进行统计学处理。结果:脑瘫组(中药洗浴组及水疗组)与正常组相比,在MV、PI、RI三方面具有统计学差异(P<0.05)。脑瘫组各组患儿肱动脉MV、TAVM、PSV三项较治疗前有明显统计学差异(P<0.05);MV:中药洗浴组2、3治疗后MV与其他各组疗后相比有具有统计学差异(P<0.05),TAVM:中药洗浴组2治疗后TAVM与其他各组疗后相比具有统计学差异(P<0.05),PSV:中药洗浴组2、3、水疗组3治疗后PSV与其他各组疗后相比具有统计学差异(P<0.05)。结论:水温38℃～40℃组中药洗浴疗法在改善MV、TAVM、PSV上较其他各组更为明显,38℃水疗在PSV改善上优于其他各组,中药洗浴水温应以38℃～40℃,水疗温度38℃为佳。Objective ：TO explore the herbal fumigation on the humerus artery blood flow of children of cerebral palsy and deter- mine the best bath temperature. Methods：We chose 120 children with CP in Pediatrics of Affiliated Hospital of Liaoning University of Traditional Chinese Medicine. We used the color doppler ultrasound to observe the mean velocity and peak systolic velocity and end-diastolic velocity and acceleration time and. resistance index and pulsatility index of the humerus artery blood flow before and after herbal fumigation and hydrotherapy. And compared with the normal group, the statistical processing was carried on. Results： Cerebral palsy group（ herbal fumigation and hydrotherapy）and normal group had a statistics difference in MV, PI and RI（ P 〈 0. 05 ）. Before and after treatment the CP group had a statistics difference in the MV,TAVM and PSV（P〈 0.05）. MV：Compared with other groups, the herbal fumigation group 2,3 had a statistics difference after treatment （ P 〈 0.05 ）. TAVM ： Compared with other groups, the herbal fumigation group 2 had a statistical difference after treatment （ P 〈 0.05 ）. PSV ： Compared with other groups ,the herbal fumigation group 2,3 and hydrotherapy group 3 had a statistical difference after treatment （P 〈 0.05 ）. Conclu- sion：Compared with other groups ,the herbal fumigation group of water temperature is 38 ~C ~ 40 ~C and has obviously improved MV ,TAVM and PSV. Compared with other groups, the hydrotherapy group of water temperature is 38 ℃ and has an obvious im- provement in the PSV. So the herbal fumigation water temperature should be 38 ℃ ~ 40 ℃ and the hydrotherapy water tempera- ture should be 38
