摘 要:目的:探讨膝关节软骨损伤的MRI表现,以提高对软骨损伤的认识及诊断能力。材料与方法:回顾性分析经关节镜或手术证实的膝关节软骨损伤21例,对其MRI表现进行总结分析,包括关节软骨、软骨下低信号线及软骨下骨的改变,并与关节镜和手术对照分析。结果:本组病例共23处关节软骨损伤。分布:髌骨6处,股骨外侧髁7处,股骨内侧髁7处,胫骨平台3处。关节软骨损伤表现:裂隙状骨折4例,软骨局部缺损10例,软骨全层缺损9例。其他:关节内软骨游离体2例,软骨损伤附近骨挫伤、水肿20例,股骨内外髁骨折3例,胫骨平台骨折2例。结论:MRI能准确显示膝关节软骨损伤的部位、范围和类型,从而对临床治疗方案的选择提供重要的依据。Purpose: To investigate the value of MR imaging of cartilage fracture of the knee joint, as well as in evaluation of clinical significance. Materials and Methods: The MRI changes of cartilage, subcartilage low signal line and sub- cartilage bone were analysed retrospectively in 21 patients with acute cartilage fracture confirmed by arthroscopy. And every manifestation on MRI and that of arthroscopy and operation was compared. Results: twenty-three sites of 21 patients include 6 patella, 7 femoral lateral condyle, 7 femoral medial condyle and 3 tibial plateau. Three types pure cartilage fracture were observed, fissuring fracture in 4 sites, superficial defect of the cartilage in 10 sites, totally defect of the cartilage in 9 sites. Corpus liberum was found in 2 patients' knee joints by MRI. Bone bruise was detected in 20 patients, and subcartilage low signal lines were normal. Conclusion: MRI is the best non-invasive method for studying cartilage fracture, can show the position, extension and types of the cartilage fracture of the knee joint and has important guiding role for clinical treatment options.
