微柱凝集法诊断自身免疫性溶血性贫血的应用价值

摘　　要：目的探讨微柱凝集法在抗人球蛋白试验中的特点及其诊断自身免疫性溶血性贫血(AIHA)的应用价值。方法对40例疑似AIHA的患者采用玻璃珠微柱凝集(CAT)技术进行检测,并用Coomb′s试验试管法(CTT)作为对照,探讨CAT技术诊断AIHA的应用价值。结果在直接抗人球蛋白试验(DAT)中,CAT法阳性39例,CTT法阳性19例;间接抗人球蛋白试验(IAT)中,CAT法阳性5例,CTT法阳性1例。两种检测方法比较,差异有统计学意义(P<0.05)。结论抗人球蛋白试验CAT法操作简便,重复性好,灵敏度高于CTT法,而DAT凝集效价在2+及其以上时对AIHA的实验室诊断有更好的应用价值。Objective To investigate the characteristics of the microcolumn agglutination test in the anti-human globulin test and its application value in diagnosing autoimmune hemolytic anemia（AIHA）.Methods 40 patients with clinically suspected AIHA were detected by glass bead microcolumn agglutination（CAT） with the Coom＇s test tube method（CTT） as the control for investigating the application value of CAT for diagnosing AIHA.Results In the direct antiglobulin test（DAT）,there were 39 cases of positive CAT and 19 cases of positive CTT;in IAT,there were 5 cases of positive CAT and 1 case of positive CTT.There was statistical difference between the CAT and CTT methods（P〈0.05）.Conclusion CAT mthod in Coomb′s test is simple to operate with good reproducibility and higher sensitivity than the CTT method.Moreover,DAT with the agglutination titre ≥2 has better application value for the laboratory diagnosis of AIHA.
