关节镜膝关节(含后关节腔)游离体取出术43例临床分析

摘　　要：目的探讨和评价关节镜膝关节(含后关节腔)游离体取出的手术治疗方法及效果。方法对2010年1月至2012年12月收治的43例膝关节(含后关节腔2例)游离体患者采取关节镜手术,行游离体取出,若并发骨关节炎,术中先进行关节清理,若并发半月板损伤,术中先进行关节镜半月板修整成形术或缝合修复术。后关节腔游离体取出术需辅助后内入路。结果 43例患者术后全部得到随访,膝关节疼痛大部分得到缓解,关节屈伸功能明显改善,无一例患者发生感染,其中1例患者术后出现反复关节积液,经抽液、对症等处理1个月后缓解。结论关节镜膝关节(含后关节腔)游离体取出术创伤小,效果确切,不良反应小,值得推荐。Objective To investigate and evaluate the methods and effects of arthroscopic removal of the loose bodies from the knee joint including the posterior compartment.Methods From January 2010 to December 2012,43 cases of loose bodies in knee joint including the posterior compartment were diagnosed and removed by arthroscopy.If the patients were complicated with osteoarthritis,joint debridement was firstly performed during operation;if the patients were complicated with meniscus injury,meniscal repair plasty or suture repair by arthroscopy were firstly conducted during operation.Arthroscopic removal of loose bodies in the the posterior compartment of knee joint needed to be assisted with posteromedial approach.Results 43 cases all were followed up after operation.In the majority of the patients,knee joint pain was alleviated significantly,and the knee joint function of flexion and extension was improved obviously.No infection happened.1 case had repeated hydrops articuli after operation and the symptoms were relieved in 1 month after draining and symptomatic treatment.Conclusion Arthroscopic removal of loose bodis in the knee including the posterior compartment has small wound,definite effect and little adverse reactions,which is worthy of promotion.
