摘 要:目的 观察改水降碘防治地方性高碘甲状腺肿效果。方法 6年前在病区自然村河南省台前县林楼村打深井 1处改水降碘。6年后观察居民甲状腺肿大率、尿碘、甲状腺吸 1 31 I率和血清 T3、T4、Tg Ab、Tm Ab水平。结果 居民生活饮用水碘含量由 42 0~ 2 0 0 0μg/ L降至 110μg/ L。6年后居民甲状腺肿患病率由 2 1.15 %降至 8.2 % ,尿碘由 (92 4.9± 6 91.8) μg/ L 降至 (172 .0± 83.1) μg/ L,2 4h甲状腺吸 1 31 I率由 (13.3± 5 .8) %提高到 (2 8.6± 9.7) %。血清 Tg Ab、Tm Ab水平也有显著变化。结论 在饮水型地方性高碘甲状腺肿病区 ,采用改水降碘的办法能有效地控制高碘甲状腺肿的流行。Objective To observe the effects of endemic goiter control by water improvement to reduce iodine.Methods In Linlou village of Taiqian county,one drinking-water project was built for improving water to reduce iodine 6 years ago.Gioiter rate,urine iodine,thyroid uptake 131 I rate,and the levels of serum T_3,T_4,TgAb and TmAb were investigated.Results Iodine concentration of drinking water decreased from 420~ 2 000 μg/L to 110 μg/L.After 6 years,goiter rate decreased from 21.15% to 8.2%,urine iodine from 924.9± 691.8 μg/L to (172.0±83.1) μg/L,and thyroid uptake 131 I rate of 24 hours improved from 13.3%±5.8% to 28.6%±9.7%.The levels of serum TgAb and TmAb were also changed greatly.Conclusions The method of improving water to reduce iodine can effectively prevent and control the prevalence of endemic goiter in iodide goiter area of drinking water type.
